TİROİD PAPİLLER KARSİNOM TANISI ALMIŞ HASTALARDA SERUM İSTHMİN-1 DÜZEYLERİNİN İNCELENMESİ

Abstract

Tiroid kanseri, tüm maligniteler içerisinde %1?lik bir oranı kapsasa da dünya genelinde yıllık yaklaşık 600.000 yeni vaka teşhis edilmektedir. İnsidansı her geçen gün artsa da papiller karsinom genel olarak iyi bir prognoza sahiptir. Yine de geç tanı alan ileri evre tiroid kanserlerinde mortalite oranı yaklaşık 6-8/1.000.000?dir.Yaptığımız çalışmada papiller karsinom tanılı hastalar ile multinodüler guatr tanılı hastalarda serum isthmin-1(ISM1) molekülün seviyeleri karşılaştırılarak tanı, takip ve cerrahi kararı vermede yol gösterici bir molekül olup olamayacağı araştırıldı. Çalışmamızda elde ettiğimiz veriler, hastane veri tabanına kayıtlı elektronik dosyaların retrospektif olarak incelenmesiyle elde edilmiştir. Araştırmamıza, 2022-2025 yılları arasında Fırat Üniversitesi Hastanesi Genel Cerrahi Anabilim Dalı'nda total tiroidektomi operasyonu yapılan 68 hasta ve kontrol grubu olarak 30 sağlıklı bireyle birlikte toplamda 98 kişi dahil edilmiştir. Hastalar, nodüler guatr , papiller tiroid kanseri ve tiroid hastalığı olmayan kişilerden oluşan toplamda üç gruba ayrılmıştır. Çalışmaya dahil ettiğimiz kişilerden alınan kan örneği Fizyoloji Anabilim Dalı laboratuvarında serum İsthmin-1 seviyeleri değerlendirmeye tabi tutulmuştur. Yaptığımız çalışma neticesinde, papiller karsinomlu hastaların serum İsthmin-1 seviyeleri nodüler guatr ve hastalıksız bireylere oranlara yüksek olduğu gözlendi. Vardığımız bu sonuç isthmin-1 serum seviyelerinin erken tanı ve tedavide yolumuzu aydınlatacağı inancını oluşturmuştur. Anahtar Kelime: Tiroid kanseri, papiller tiroid kanseri, isthmin-1

Although thyroid cancer accounts for only %1 of all malignancies, approximately 600,000 new cases are diagnosed worldwide each year. Even though its incidence continues to increase, papillariy carcinoma generally has a good prognosis. Nevertheless, in advanced-stage thyroid cancers diagnosed late, the mortality rate is approximately 6-8 per 1,000,000. ?n our study, we investigated whether serum isthmin-1 (ISM1) levels could serve as a guiding biomarker for diagnosis, follow-up, and surgical decision-making by comparing patients diagnosed with papillery carcinoma and those multinodular goiter. The data in our study were obtained through a retrospective review of electronic records registered in the hospital database. A total of 98 individuals were included in our study, consisting of 68 patients who underwent total thyroidectomy between 2022 and 2025 at the Department of Genereal Surgery, Fırat University Hospital, and 30 healthy individuals as the control group. The patients were dividied into three groups; those with nodular goiter, those with papillary thyroid cancer and those without thyroid disease. Blood samples taken from the individuals included in the study were analyzed for serum isthmin-1 levels in the laboratory of the Department of Physiology. As a result of our study, it was observed that serum isthmin-1 levels in patients with papillary carcinoma were higher compared to those with nodular goiter and healthy individuals. This finding has led to the belief that serum isthmin-1 levels may shed light on early diagnosis and treatment. Keywords: Thyroid cancer, papillary thyroid cancer, isthmin-1
