VERNAL KERATOKONJONKTİVİTTE TOPİKAL FLUOROMETOLON'UN GÖZYAŞI MAKROFAJ MİGRASYON İNHİBİTÖR FAKTÖR VE İNTERLÖKİN 16 DÜZEYLERİNE ETKİSİ

Abstract

Vernal keratokonjonktivitin patogenezi tam olarak anlaşılamamış olup sadece Tip I aşırı duyarlılık cevabıyla açıklanamamaktadır. Hastalığın patogenezinde T helper 2 (Th2) hücrelerinin rol aldığı inflamatuvar cevabın da katkısı olabilir. Gecikmiş tip hipersensitive ve allerjik reaksiyonlarda rol oynadığı kanıtlanan Makrofaj Migrasyon İnhibitör Faktör (MIF) ve İnterlökin 16 (IL-16)'nın vernal keratokonjonktivitteki etkisi ile ilgili bir çalışma bulunmamaktadır.Çalışmamızda sağlıklı bireylerden oluşan kontrol grubunda ve topikal fluorometholon ile 21 günlük tedavi öncesi ve sonrası vernal keratokonjonktivitli hastalarda gözyaşı MIF ve IL-16 düzeyleri ELISA yöntemi ile incelendi. MIF ve IL-16 seviyelerinin ölçümü amacıyla kapiller tüp yardımıyla gözyaşı örnekleri biriktirildi.Kontrol grubu ile karşılaştırıldığında vernal keratokonjonktivitli hastalarda gözyaşı MIF ve IL-16 seviyeleri fluorometholon ile tedavi öncesi ve sonrası anlamlı olarak daha yüksek bulundu (p<0,05). Vernal keratokonjonktivitli hastalarda fluorometholon tedavisi sonrası tedavi öncesine göre IL-16 düzeyleri anlamlı derecede azalırken (p<0,05), MIF grubunda anlamlı bir azalma saptanmadı (p>0,05).Bu sonuçlar MIF ve IL-16'nın vernal keratokonjonktivitin patogenezinde sürece önemli bir katkısı olduğunu düşündürmektedir. Vernal keratokonjonktivit hastalarının topikal kortikosteroid ile yapılan tedavileri MIF düzeylerini azaltmazken IL-16 düzeylerini anlamlı derecede azalttığı saptanmıştır.

The pathogenesis of vernal keratoconjunctivitis is not fully understood and cannot be explained only with type I hypersensitivity reaction. The inflammatory response including T helper 2 (Th2) cells may be an additional mechanism in the pathogenesis of this disease. There is not any published text about the effect of Macrophage migration inhibitory factor (MIF) and Interleukin-16 (IL-16) in vernal keratoconjunctivitis which have the role in delayed type hipersensivity reactions and allergic reactions.In this study, tear levels of MIF and IL-16 were measured by ELISA kit using samples from healthy subjects and patients with vernal keratoconjunctivitis, before and after 21 days treatment of topical fluorometholone. Capillary tubes have been used to collect tears for the measurement of levels of MIF and IL-16.Tear levels of MIF and IL-16 in patients with vernal keratoconjunctivitis before and after the fluorometholone treatment were significantly higher than those in controls (p<0,05). In the patients with vernal keratoconjunctivitis tear level of IL-16 after the treatment was significantly lower than the levels before the treatment (p<0,05). Tear levels of MIF were not significantly different between groups (p>0,05).These results considered that MIF and IL-16 have significant effect on the pathogenetic process of vernal keratoconjunctivitis. We determined that treatment of vernal keratoconjunctivitis with topical corticosteroid causes significant decrease in IL-16 levels but have no effect on MIF levels.
