OBSESİF KOMPULSİF KİŞİLİK BOZUKLUKLU BİREYLERDE ANTERİOR SİNGULAT KORTEKS VE KAUDAT ÇEKİRDEK HACİMLERİ VE KLİNİK DEĞİŞKENLERLE İLİŞKİSİ

Abstract

Çalışmamız Obsesif Kompulsif Kişilik Bozukluğu (OKKB) olan hastalarda anterior singulat korteks ve kaudat çekirdek volumetrik ölçümlerini değerlendiren ilk çalışmadır. Bu çalışmada obsesif kompulsif kişilik bozukluğunun etyopatogenezini daha iyi anlayabilmek adına hastaların anterior singulat korteks ve kaudat çekirdek hacim değişikliklerini araştırma hedeflendi. Çalışma süresince Fırat Üniversitesi Hastanesi Psikiyatri Anabilim Dalı'na başvuran ve yatarak ya da ayaktan tedavi gören, DSM-IV-TR tanı ölçütlerine göre OKKB tanısı almış ve çalışma ölçütlerine uyan 16 hasta ile 18 sağlıklı kontrol grubu çalışmaya dahil edilmiştir. Hastalara Sosyodemografik ve Klinik Veri Formu, Beck Anksiyete Ölçeği (BAÖ), Beck Depresyon Ölçeği (BDÖ), Çok boyutlu Mükemmelliyetçilik ölçeği (FÇBMÖ), SCID I ve II uygulanmıştır. Hasta ve kontrol grubunda manyetik rezonans görüntüleme (MRG) kullanılarak sağ ve sol anterior singulat korteks ve kaudat çekirdek volümetrik ölçümleri gerçekleştirildi. Çalışmamızda anterior singulat korteks ve kaudat çekirdek volümleri açısından hastalar ve kontroller arasında anlamlı farklılıklar bulunmadı. . Sosyodemografik özellikler karşılaştırıldığında ise elde ettiğimiz sonuçlar literatürle uyumluydu. Ölçek puanları ile volümetrik ölçümler karşılaştırıldığında da anlamlı farklılıklar yoktu. Sonuç olarak obsesif kompulsif kişilik bozukluğu fizyopatolojisinde anterior singulat korteks ve kaudat çekirdekte volümetrik olarak anlamlı bulgulara rastlanılmadı. Bu bölgelerin fonksiyonel özelliklerini inceleyen görüntüleme teknikleri ve bilişsel işlevleri inceleyen testlerin birlikte ele alındığı çalışmaların daha verimli sonuçlar ortaya koyacağı düşünülmektedir. In this study, ıt was aimed to investigate the volume changes of anterior cingulat cortex and caudat nucleus in order to better understand the etiopathogenesis of obsessive compulsive personality disorder. During the study, 16 patients who applied for The Department of Psychiatry at the Hospital of Fırat University and who received inpatient or outpatient treatment and diagnosed as obsessive compulsive personality disorder according to DSM-IV-TR diagnostic criteria and who met the study criteria and 18 healthy control groups were included in the study.The patients were administered the sociodemographical data form, the Beck Anxiety Scale, the Beck Depression Scale, the Frost Multi-dimensional Perfectionism Scale, SCID I and II. Right and left anterior cingulat cortex and caudat nucleus volumetric measurements were performed in the patient and control group using magnetic resonance imaging (MRI). In our study; there were no differences between the patients and healthy controls for anterior cingulate cortex and caudat nucleus volumes. When the sociodemographic characteristics were compared, the results obtained were in accordance with the literatüre. When we compared the scale scores with the volumetric measurements, there were no any statistically significant difference. As a result, our investigation did not demonstrate any statistically significant differences for anterior cingulat cortex and caudat nucleus in OCPD pathophysiology. It is thought that the studies that are carried out together with the imaging techniques that overlook the functional properties of these regions and the cognitive function tests will give more efficient results.
