Introduction: Primary adenocarcinomas of the renal pelvis constitute one percent of all renal malignancies. It was reported that these carcinomas arise from Bellini ducti, embryonic remnants, and transitional cells of the pelvis. Materials and Methods: Left nephrectomy was performed in a 72 years old male patient. A 6.5×4.5×4 cm size tumor settled to renal pelvis was found and primary adenocarcinoma of the renal pelvis was diagnosed. Conclusion: The differential diagnosis of the primary adenocarcinoma of the renal pelvis is important because of the frequent metastasis of the other organ adenocarcinomas to the kidney.
