SNODGRASS (TIPU) TEKNİĞİ İLE HİPOSPADİAS CERRAHİSİ UYGULANAN HASTALARIN RETROSPEKTİF DEĞERLENDİRMESİ

Abstract

Hipospadias cerrahisinde temel amaç; fonksiyonel, kozmetik açıdan düzgün, cinsel ilişkiye izin veren düz bir penis ve ayakta idrar yapmayı sağlayan slit benzeri bir meatus oluşturmaktır. Bu amaçla bugüne kadar yüzlerce teknik tanımlanmış olsa da, 1994 yılında Snodgrass tarafından tanımlanan "Tubularized Incised Plate" (TIPU) tekniği, düşük komplikasyon oranları ve kozmetik başarısı nedeniyle günümüzde en yaygın kullanılan yöntem haline gelmiştir. Çalışmamızda, kliniğimizde 2012-2025 yılları arasında TIPU tekniği ile opere edilen 438 hastanın sonuçları retrospektif olarak değerlendirildi. üretral mea lokasyonu, hastaların yaşı, üretroplasti için kullanılan sütur türü, kullanılan üretral stent türü, kaç aşamalı cerrahi yapıldığı, pre-operatif hormon replasman tedavisi kullanılma durumu, üretral stent ile takip süresi ve postoperatif geç dönem komplikasyonlar değerlendirme kriteri olarak kullanıldı. Hastalar üretral mea lokasyonuna göre Hadidi sınıflaması (Tablo2, 35) kullanılarak 3 gruba ayrıldı. Grup I; anterior, Grup II; middle, Grup III; posterior yerleşimli olgulardan oluştu. TIPU tekniği distal ve mid-penil hipospadiaslarda yüksek başarı oranları ile güvenle uygulanabilir. Ancak proksimal vakalarda komplikasyon oranlarının arttığı unutulmamalı ve aileler bu konuda bilgilendirilmelidir. Çalışmamızın verileri ışığında; post-operatif dönemde meatal stenozun önlenmesi fistül riskini belirgin şekilde azaltmaktadır. Ayrıca üretroplasti için Foley sonda kullanımı meatal stenoz riskini ciddi oranda artırdığından, rutin kullanımda feeding kateter gibi daha sert yapılı kateterlerin tercih edilmesi cerrahi başarıyı artıracaktır. The primary objective of hypospadias surgery is to construct a functional, cosmetically acceptable straight penis that allows for sexual intercourse and a slit-like meatus enabling voiding in a standing position. Although hundreds of techniques have been described to date for this purpose, the "Tubularized Incised Plate" (TIPU) technique, described by Snodgrass in 1994, has become the most widely used method today due to its low complication rates and cosmetic success. In our study, the results of 438 patients who were operated on with the TIPU (Tabularized Incised Plate Urethroplasty) technique in our clinic between 2012 and 2025 were retrospectively evaluated. The location of the urethral meatus, patient age, type of suture used for urethroplasty, type of urethral stent used, number of surgical stages, preoperative hormone replacement therapy status, duration of follow-up with a urethral stent, and late postoperative complications were used as evaluation criteria. Patients were divided into three groups according to the Hadidi classification (Table 2, 35) based on the location of the urethral meatus. Group I consisted of cases with anterior localization, Group II with middle localization, and Group III with posterior localization. The TIPU technique can be safely performed with high success rates in distal and mid-penile hypospadias. However, it should be noted that complication rates increase in proximal cases, and families should be informed accordingly. In light of our study data, the prevention of meatal stenosis in the postoperative period significantly reduces the risk of fistula. Furthermore, since the use of Foley catheters for urethroplasty significantly increases the risk of meatal stenosis, preferring catheters with a stiffer structure, such as feeding catheters, in routine practice will increase surgical success.
