TEMPORAL LOB EPİLEPSİSİ OLAN HASTALARDA PERFÜZYON MR DEĞERLENDİMESİ VE NORMAL POPULASYONLA KIYASLANMASI

Abstract

EEG ile fokal odak saptanan temporal lob epilepsili olgularda bu alandaki perfüzyon degisikliklerinin rCBV, rCBF ve MTT parametreleri kullanılarak hesaplanması ve normal kontrol grubu ile kıyaslanması amaçlanan çalısmada EEG ile temporal odak saptanan 21 olgu ve bas agrısı ön tanısıyla beyin MRG tetkiki istemi yapılan 21 kontrol grubu kullanılmıstır. Epileptik odaktaki perfüzyon degisikliklerini saptamak amacıyla çalısma grubunda postiktal dönemdeki ilk 3 saat içerisinde ölçümler yapılmıstır. Dynamic susceptibility contrast (DSC) teknigi kullanılarak elde edilen görüntüler is istasyonunda islenerek her iki hipokampüste rCBV, rCBF ve MTT degerleri hesaplanmıstır. rCBV ve rCBF degerlerinin çalısma ve kontrol grubunda sag ve sol hipokampüs arasında anlamlı farklılık göstermedigi izlenmistir (P>0.05). Bunun nedeni çalısma grubumuzdaki patolojilerin çogunlugunu bilateral mezial temporal skleroz'un olusturmasıdır. 21 olgunun 19 unda bilateral mezial temporal skleroz, 2'sinde sag temporal lobdan kaynaklı epileptik odak saptanmasıdır. Ancak çalısma grubu ile kontrol grubu arasında yapılan karsılastırmada çalısma grubunda hipokampüslerde rCBV ve rCBF degerlerinde kontrol grubuna göre artıs oldugu dikkati çekmistir (P<0.05). MTT degerinde ise hasta ve kontrol grubu arasında anlamlı degisiklik izlenmemistir (P>0.05). Çalısma grubunda kontrol grubuna göre MTT degeri artmazken rCBV ve rCBF degerlerinin artısı postiktal dönemde epileptik odagın bulundugu bölgelerde vazodilatasyon gelistigini düsündürmüstür. The patients with temporal lobe epilepsy were diagnosed focal focus with EEG and the perfusion changes which were calculated by using rCBV, rCBF, MTT and compared with normal control group. Twenty-one cases were diagnosed of temporal focus with EEG and twenty-one patients who had without neurologic complaints except for headache were included in to study. The measurement was made at working group in postictal period in three hours. The images obtained by using dynamic suspectibility contrast were performed at workstation and rCBV, rCBF and MTT values were calculated at both hipocampus. The rCBV and rCBF values doesn?t increase compared with control group on the right and left of hipocampus (P>0.05), because the majority of pathologies in our study group had formed bilateral mesial temporal sclerosis. In 29 patients of 21 patients diagnosed bilateral mesial temporal sclerosis, the other two patients were diagnosed as epileptic focus releated to right temporal lobe. The MTT doesn?t increase compared with control group but rCBV and rCBF values increased compared with control group. It was thought that increase of rCBV and rCBF values in postictal period on the epileptic focus duo to the vasodilatation.
