KOLOREKTAL KANSERLERDE PROGNOSTİK FAKTÖRLERİN BELİRLENMESİ

Abstract

Kolorektal kanserler dünyada dördüncü sıklıkta görülen kanser türü olup 1990 yılında 783 000 yeni vaka bildirilirken 2002 yılında 1 milyon yeni vaka bildirilmiştir. Sağkalım çalışmalarına göre 5 yıllık sağkalım Kuzey Amerika'da % 65, Batı Avrupa'da % 54, Doğu Avrupa'da % 34 ve Hindistan'da % 30'dur. Son yarım yüzyıl içerisinde yapılan çalışmalarda pek çok faktörün kolorektal kanserli hastalarda sağkalım ile ilişkisi olduğu öne sürülmüştür.1994-2005 yılları arasında kolorektal kanser nedeniyle opere edilmiş 126 kolorektal kanserli hastaya ait veriler retrospektif olarak incelendi. Hastaların 72 (% 57,1)'si erkek, 54 (% 42,9)'ü kadın idi. Hastaların yaş ortalaması 57,5±15,2 idi. Medyan takip süresi 44,5 ay, medyan genel sağkalım 60 ay ve medyan hastalıksız sağkalım 40 ay idi. 29 (% 23,0) hastada metastaz ve/veya lokal nüks geliştiği belirlendi. Metastatik hastalarda medyan genel sağkalım 10 ay olarak bulundu.Tek değişkenli analizde yaş, histolojik tip, patolojik evre, tümörün evresi, barsak duvarı penetrasyonu, lenf nodu tutulumu, uzak organ metastazı, cerrahi sınır, preoperatif CEA düzeyi ve uygulanan cerrahinin tedavi GSK süreleri ile anlamlı ilişkisi bulundu. Çok değişkenli analizde ise tümörün evresi, barsak duvarı penetrasyonu, bölgesel lenf nodu tutulumu ve uzak organ metastazı GSK süreleri ile anlamlı ilişkisi bulunan bağımsız prognostik parametreler olarak bulundu.Tek değişkenli analizde patolojik evre, tümörün evresi, barsak duvarı penetrasyonu, bölgesel lenf nodu tutulumu, uzak organ metastazı, cerrahi sınır, preoperatif CEA düzeyi ve uygulanan cerrahi tedavinin HSK süreleri ile anlamlı ilişkisi bulundu. Çok değişkenli analizde tümörün evresi, barsak duvarı penetrasyonu, uzak organ metastazı ve cerrahi sınır HSK süreleri ile anlamlı ilişkisi bulunan bağımsız prognostik parametreler olarak bulundu.Çalışmamızda da görüldüğü gibi, kolorektal kanserli hastaların gidişatının tam olarak tahmini oldukça güç bir klinik sorundur. Bizim çalışmamız, retrospektif ve göreceli olarak az sayıda hasta ile yapılmıştır. Bu nedenle, belirlenen konularda güncel klinik uygulamayı etkileyecek kesin sonuçların çıkarılması için çok daha fazla sayıda hastayı içeren prospektif çalışmanın yapılması gerekmektedir.Colorectal cancer is the fourth commonest form of cancer occurring worldwide, with an estimated 783 000 new cases diagnosed in 1990 and 1 million new cases in 2002. Survival estimates at 5 years are 65% in North America, 54% in Western Europe, 34% in Eastern Europe, and 30% in India. In the past 50 years, claims have been made of numerous variables being related to survival in patients with colorectal cancer.One hundred twenty six patients, operated with colorectal cancer in our clinic between the years 1994 and 2005 were investigated. 72 (57,1 %) of the patients were men and 54 (42,9 %) were woman. The mean age was 57,5±15,2 years. Median follow-up time was 44,5 months, median overall survival was 60 months and median disease free survival was 40 months.. In 29 (23,0 %) patients, metastasis and/or local recurrence were detected. In metastatic patients median overall survival was found as 10 months.According to the univariate analysis, the factors influencing overall survival rate were the following: age, hystologic type, pathologic grade, stage of tumor, bowel wall penetration, regional lymph nodes metastasis, distant metastasis of tumor, surgical magrin, preoperative CEA levels and type of surgery. The following variables were independent prognostic factors for overall survival as determined by multivariate analysis: stage of tumor, bowel wall penetration, regional lymph nodes metastasis, metastasis of tumor, and distant metastasis of tumor.According to the univariate analysis, the factors influencing disease free survival rate were the following: pathologic grade, stage of tumor, bowel wall penetration, regional lymph node metastasis, distant metastasis of tumor, surgical magrin, preoperative CEA levels and type of surgery. The following five variables were independent prognostic factors for disease free survival as determined by multivariate analysis: stage of tumor, bowel wall penetration, distant metastasis of tumor surgical magrin.Our study suggest that accurate prediction of the outcome of colorectal cancer patients is a complex clinical problem. Despite numerous studies performed worldwide, the accurate determination of prognostic factors for colorectal cancer remains unsuccessful. Our study was done retrospectively and respectively with small number of patients. Therefore, further prognostic and large studies are needed to accurately determine the role of the various clinical and pathological factors in colorectal cancer prognosis.
