ABORTUS İMMİNENS TANISIYLA TAKİP EDİLEN GEBELERDE PLAZMA KİSSPEPTİN DÜZEYLERİNİN ARAŞTIRILMASI

Abstract

Abortus imminens 20. gebelik haftasından önce vajinal kanama ve batında kramp tarzında ağrı ile bulgu veren ve yaklaşık yarısında da abortus gerçekleşen bir klinik patolojidir. Başarılı bir embriyolojik gelişim için implantasyon anahtar basamaklardan birisidir. Kisspeptin (Kp) peptit yapıda bir hormondur ve trofoblast invazyonunu engellediği gösterilmiştir. Kisspeptin-10 (Kp-10) en güçlü trofoblast invazyon inhibisyonu etkisine sahip olan kisspeptindir. Bu çalışma; abortus imminensli gebeler ile sağlıklı gebelerde Kp-10'un gebelik dönemindeki plazma seviyelerinin belirlenmesi ve bu hormonun abortus imminens etyolojisindeki olası potansiyel rolünün açıklığa kavuşturulması amacıyla planlanmıştır.Çalışmaya abortus imminensli 20 gebe ve tamamen sağlıklı 20 gebe dahil edildi. Olguların tümünden ilk 20 gebelik haftasında Kp-10düzeylerini belirlemek için venöz kan örnekleri alındı. Hormonlar ELİSA yöntemi ile analiz edildi. Sonuçların değerlendirilmesinde Independent Samples testi kullanıldı. Çalışılan parametrelerin birbirleriyle ilişkisi Pearson korelasyon analizi ile degerlendirildi. P 0.05'ten küçük olması istatistiksel olarak anlamlı kabul edildi.Çalışmamızda; abortus imminensli hasta grubunda plazma Kisspeptin-10 (414±211 pg/mL) düzeyleri, kontrol gebe grubuna (6240±2042 pg/mL) göre istatistiksel olarak anlamlı düzeyde düşük bulundu (p<0.05).Sonuç olarak; Kp-10'un abortus imminens patofizyolojisinde rol oynadığı düşünülmüştür.Anahtar Kelimeler: Abortus İmminens, Kisspeptin-10, trofoblast, implantasyon

Clinical diagnosis of imminent abortion is considered when bloody vaginal discharge or bleeding and like cramp pain in pelvic is seen before the twentieth week and in this pathology half percent abortion occurred. The implantation is one of key steps for a successful embryonic development. Kisspeptin (Kp) is a peptide hormone and studies indicated that it prevents trophoblast invasion. In all kisspeptins, Kp-10 posses the strongest effect of invasion inhibiting function.This study was to clarify the plasma levels of Kp-10 in women with imminent abortion and healthy pregnant women during pregnancy and to explain its possible role in etiology of imminent abortion. 20 patients with imminent abortion and 20 healthy pregnant women were included in this study. For determination Kp-10 level, venous blood samples were collected from all of the cases at the first 20 weeks of pregnancy. Hormones were analyzed by using ELISA method. Independent Samples test was used for evaluation of outcomes. Pearson correlation coefficients were used to evaluate the correlation between parameters. P values less than 0.05 were accepted as significant.Kisspeptin-10 levels in group with imminent abortion (414±211 pg/mL) was significantly lower compared with kisspeptin levels in normal group (6240±2042 pg/mL), (p<0.05).In conclusion, it was thought that Kp-10 could have a role in the pathophysiology of imminent abortion.Keywords: Abortion imminent, Kisspeptin?10, trophoblast, implantation
