ALOPESİ AREATA HASTALARINDA TOPİKAL KORTİKOSTEROİD (%0.05 KLOBETASOL PROPİONAT) TEDAVİSİ İLE % 1 TOPİKAL PİMEKROLİMUS TEDAVİSİNİN KARŞILAŞTIRILMASI

Abstract

Alopesi areata, herhangi bir eritem, skuam olmaksızın lokalize alanlarda ortaya çıkan, sikatris bırakmayan saç dökülmesidir. Bu çalışmada alopesi areatalı hastalarda %0.05 klobetasol propiyonat ile %1 pimekrolimus tedavi etkinlikleri değerlendirilmeye çalışılmıştırÇalışmaya alopesi areata tanısı koyulan 100 hasta alındı. Hastalar 4 gruba ayrıldı. Otuz hastaya %1 pimekrolimus krem, 30 hastaya %0.05 kolbetasol propiyonat krem, 20 hastaya ise plasebo grubu olarak katı pür vazelin tedavisi başlandı. Yirmi hastada ise saçlı deri iki eşit parçaya bölünerek bir tarafa %1 pimekrolimus krem, diğer tarafa ise %0.05 klobetasol propiyonat krem tedavileri başlandı.Hastalara uygulanan mevcut tedaviler sonrasında, iyileşme yüzdesi ve iyileşme skorları hesaplandı. %1 pimekrolimus grubunda iyileşme yüzdesi 53.73 ? 44.49, iyileşme skoru 3.63 ? 2.07; %0.05 klobetasol propiyonat grubunda iyileşme yüzdesi 47.00 ? 44.80, iyileşme skoru 3.33 ? 2.20; plasebo grubunda iyileşme yüzdesi 35.50 ? 40.53, iyileşme skoru 2.75 ? 1.88; kıyas grubunda ise %1 pimekrolimus krem tedavisi sonucunda iyileşme yüzdesi 48.25 ? 41.14, iyileşme skoru 3.40 ? 1.98; %0.05 klobetasol propiyonat krem tedavisi sonucu ise iyileşme yüzdesi 61.75 ? 39.31, iyileşme skoru ise 4.05 ? 1.84 olarak bulunduÇalışmamız sonucunda, topikal steroidlerin uzun süreli kullanımlarında görülebilen yan etkilerin, %1 pimekrolimus krem tedavisinde ortaya çıkmamasını, uzun dönem tedavi gerekebilen alopesi areata açısından bir avantaj olarak değerlendirmekteyiz. Ayrıca alopesi areatalı hastalarda alopesik plaklarda aktivite varlığında %1 pimekrolimus krem tedavisinin daha etkili olduğunu saptadık. Özellikle retiküler tarzda dökülmesi olan aktif plaklarda %1 pimekrolimus krem tedavisi ile iyi sonuçlar alınabileceğini düşünmekteyiz.

Alopecia areata (AA) is a nonscarring hair loss. There is no erythema and scale on the local areas of scalp. We aimed the comparasion of %0.05 clobetasol propionate and %1 pimecrolimus efficiency in AA patients.The study included 100 AA patients. Patients were diveded to 4 groups. 30 patients used %1 pimecrolimus cream, 30 patients used %0.05 clobetasol propionate cream, 20 patients used petrolatum as plasebo. Scalp was diveded to two equal pieces at the 20 patients. They used %1 pimecrolimus cream to one side and %0.05 clobetasol propionate cream to the other side.At the end of the treatment, healing percentages and healing scores were calculated. İn the %1 pimecrolimus group; healing percentage was 53.73±44.49, healing score was 3.63±2.07. İn the %0.05 clobetasol propionate group; healing percantage was 47.00 ? 44.80, healing score was 3.33 ? 2.20. İn the placebo group; healing percantage was 35.50 ? 40.53, healing score was 2.75 ? 1.88. İn the comparison group; %1 pimecrolimus healing percantage was 48.25 ? 41.14, healing score was 3.40 ? 1.98, %0.05 clobetasol propionate healing percantage was 61.75 ? 39.31, healing score was 4.05 ? 1.84.At the end of the study we did not see side effects at %1 pimecrolimus cream as we saw at long term topical corticosteroid treatment. We think that this is an advantage for long term needed treatments in AA. İn addition to this we found that %1 pimecrolimus cream is more effective in activity (+) AA patients. We think that %1 pimecrolimus cream is effective at especially reticular type of AA.
