PSORİASİS VULGARİSLİ HASTALARDA VE BEHÇET HASTALARINDA PREPTİN VE AMİLİN DÜZEYLERİNİN ARAŞTIRILMASI

Abstract

Psoriasis, kronik inflamatuar hastalıklar içerisinde en sık görülenidir. Etyopatogenezinde birçok genetik ve çevresel faktör rol oynamaktadır. Behçet hastalığı kronik seyirli, tekrarlayıcı, sistemik bir vaskülittir. Hastalığın etyoloji ve patogenezi tam olarak bilinmemektedir.Psoriasis ve Behçet hastalarında görülen inflamasyonun insülin direncinde artışa ve metabolik sendrom bozukluğuna yol açtığı ileri sürülmektedir. Preptin ve amilin insülin direnci ile ilişkili peptidler olup pankreasın adacık hücrelerinde insülin ile birlikte salgılanırlar. Bu çalışmada preptin ve amilinin psoriasis, Behçet hastalığı ve metabolik sendrom ile olan ilişkisinin obeziteden bağımsız olarak araştırılması amaçlanmıştır.Çalışmaya 30 psoriasisli hasta ile 30 Behçet hastası ve kontrol grubu olarak 30 sağlıklı kişi alındı. Tüm katılımcıların açlık kan şekeri, trigliserid, LDL, VLDL, HDL, total kolesterol, HbA1c, C-peptid, insülin, serum preptin ve amilin düzeyleri ölçüldü. Ayrıca hasta grupları ve kontrol grubunun vücut kitle indeksi, bel çevresi, kan basıncı ölçümleri yapıldı ve HOMA-IR değerleri hesaplandı.Psoriasis ve Behçet hastalarında serum preptin düzeyi kontrol grubuna göre istatistiksel olarak anlamlı derecede düşük bulundu (sırasıyla p<0.001, p=0.004). Psoriasis ve Behçet hasta grubunda serum amilin seviyeleri kontrol grubuna göre istatistiksel olarak anlamlı derecede düşük bulundu (sırasıyla p=0.004 ve p=0.008). Psoriasisli hastalarla Behçet hastaları arasında serum preptin ve amilin seviyeleri açısından anlamlı bir farklılık saptanmadı (p>0.05). Psoriasis hasta grubunda serum insülin düzeyi ve HOMA-IR indeksi kontrol grubuna göre anlamlı derecede düşük olarak belirlendi (sırasıyla, p=0.02, p=0.03).Psoriasisli hastaların 8?inde (%26.7), Behçet hastalarının 5?inde (%16.7) metabolik sendrom mevcuttu. Metabolik sendromu olan hastalarda metabolik sendromu olmayanlara oranla serum preptin düzeyi düşük olarak saptandı (p=0.01).Sonuç olarak biz bu çalışmada psoriasisli hastalarda ve Behçet hastalarında serum preptin ve amilin düzeylerinin sağlıklı kontrollere oranla düşük olduğunu saptadık. Preptin ve amilin düzeylerinin psoriasis ve Behçet hastalığı ile ilişkili olmadığını; fakat metabolik sendrom ile ilişkili olabileceğini düşünmekteyiz.Anahtar Kelimeler: Psoriasis, Behçet hastalığı, insülin direnci, preptin, amilin.

Psoriasis is the most frequent type in the chronical inflammatory diseases. Many genetic and environmental factors play a significant role in etiopathogenesis of psoriasis. Behcet disease is a chronical, recurrent, systemic vasculitis. The etiology and pathogenesis of Behcet disease is not known completely.It is suggested that inflammation, which is seen in the diseases of psoriasis and Behcet, causes a rise in insulin resistance and metabolic syndrome disorder.Preptin and amylin are peptids related to insulin resistance, and they are released with insulin in islet-cells of pancreas. In this study, it was aimed to investigate the relation of preptin and amylin to the psoriasis, Behcet diesease and metabolic syndrome, independently free from obesity.Thirty patients with psoriasis and 30 patients with Behcet disease and 30 healthy people as control group were taken to this study. The levels of fasting blood glucose, trigliserid, LDL, VLDL, HDL, total cholesterol, HbA1c, C-peptid, insulin, serum preptin and amylin of all participants were measured. In addition, body mass index, waist circumference, blood pressure measurements of patient groups and control group were done and values of HOMA-IR were calculated.The serum preptin level of psoriasis and Behcet patients was found as significantly lower than the level of the control group ( respectively, p<0.001, p=0.004). The serum amylin levels of psoriasis and Behcet patients were found as significantly lower than the level of the control group ( respectively, p=0.004 and 0.008). There was no important difference in terms of level of serum preptin and amylin between the patients with Behcet and psoriasis( p>0.05). The serum insulin level and HOMA-IR index of psoriasis patients group were found as significantly lower than control group ( respectively, p=0.02, p=0.03 ).Eight psoriasis patients (%26.7), five Behcet patients (%16.7) had metabolic syndrome. Serum preptin level of patients with metabolic syndrome was found as significantly lower than the non-metabolic syndrome patients.In conclusion, in this study, we determinated that serum preptin and amylin levels were lower in patients with psoriasis and Behcet compared to healthy controls. viWe think that preptin and amylin levels are not associated with psoriasis and Behcet disease, but may be they are associated with metabolic syndrome.Keywords: Psoriasis, Behçet disease, insulin resistance, preptin, amylin
