ORTOPEDİK ALT EKSTREMİTE CERRAHİSİNDE, POSTOPERATİF ANALJEZİ İÇİN İNTRAVENÖZ VERİLEN DEKSKETOPROFEN TROMETAMOL'UN OPTİMAL DOZUNUN ARAŞTIRILMASI

Abstract

Ortopedik alt ekstremite cerrahisinde, ASA I-II olan hastalara, postoperatif analjezi için intravenöz verilen deksketoprofen trometamol'un optimal dozunu bulmayı amaçladık.Bu amaçla 60 hastayı rastgele 4 gruba ayırdık ve doz rejimlerini Grup I'e 1 mg/kg tramadol HCl yükleme dozu, HKA ile 25 mg bolus tramadol HCl, Grup II'ye 50 mg deksketoprofen trometamol I.V. (25 mg 2x1), HKA ile 25 mg bolus tramadol HCl, Grup III'e 75 mg deksketoprofen trometamol I.V. (25 mg 3x1), HKA ile 25 mg bolus tramadol HCl ve Grup IV'e 100 mg deksketoprofen trometamol I.V. (50 mg 2x1), HKA ile 25 mg bolus tramadol HCl olacak şekilde belirledik. Bulantı-kusma skalası 2 ve üzerinde olan veya bulantıyı tolere edemeyen ve antiemetik isteyen olgulara 10 mg I.V metoklopramid, dispepsi gibi diğer gastrointestinal yan etkiler için ise 40 mg famotidin verildi, ilaçların ilk dozları operasyonun bitmesine 30 dakika kala yapıldı. HKA cihazı ile grupların tümünde kilitli kalma süresi 30 dakika olarak ayarlandı, dört saatlik maksimum doz 100 mg olacak şekilde uygulandı.Derlenme odasında, 1., 6., 12. ve 24. saatlerde hemodinamik ve solunumsal parametreler, postoperatif ağrı skoru, sedasyon skoru, bulantı-kusma ve kullanılan tramadol dozu açısından grupları karşılaştırdık.Sonuç olarak postoperatif ağrı kontrolünde HKA ile birlikte kullanılan, 100 mg deksketoprofen trometamol, istenilen düzeyde analjezi, en az yan etki ve arzulanan hasta memnuniyeti açısından kullanılabilir doz rejimidir.Anahtar Kelimeler: Postoperatif ağrı, hasta kontrollü analjezi, tramadol HCl., deksketoprofen trometamol

We aimed to determine optimum dosage of intravenous administrated dexketoprofen trometamol HCl at postoperative period for postoperative analgesia in orthopedic lower extremity surgery at ASA I-II patientsWe divided the patients in to 4 groups randomly and dosage regimens were determined like following doses: Group I 1 mg/kg tramadol HCl loading dose, 25 mg tramadol HCl bolus dose with patient controlled analgesia (PCA), group II received 50 mg dexketoprofen trometamol I.V. (25 mg 2x1) 25 mg bolus tramadol HCl with PCA, Group III received 75 mg deksketoprofen trometamol I.V. (25 mg 3x1), 25 mg bolus tramadol HCl with PCA, Group IV received 100 mg deksketoprofen trometamol I.V. (50 mg 2x1), 25 mg bolus tramadol HCl with PCA. The patients who vomitting neousea scale was more than 2 or the patients could not tolerate nausea and demand antiemetic drugs was administereted 10 mg I.V metoklopramid. 40 mg famotidin administrated for dyspepsia or other gastrointestinal side effects. First doses of drugs administrated before 30 minutes the end of the operation. Lock in period of the PCA device was programmed as 30 minutes and the maximum doses as 100 mg for four hours.We compared the groups for hemodynamic and respiratory parameters, postoperative pain score, sedation score nausea-vomitting and consumed tramadol HCl doses at 1., 6., 12. and 24. hours in the recovery room.In conclusion, for postoperative pain management, PCA with 100 mg deksketoprofen trometamol HCl is suitable dosage regimen to obtain desired level of analgesia, minimum advers effect and desired patient satisfaction.Key words: postoperative pain, patient controlled analgesia, tramadol HCl, dexketoprofen trometamol
