LOKALİZE PROSTAT KANSERİ TEDAVİSİNDE ÜÇ BOYUTLU LAPAROSKOPİNİN YERİ

Abstract

Prostat kanseri, erkeklerde ikinci en sık görülen kanserdir. Lokalize prostat kanserinde altın standart tedavi yöntemi radikal prostatektomidir. Günümüzde radikal prostatektomi ameliyatı açık (retropubik, perineal), laparoskopik ve robot yardımlı laparoskopik yöntemler ile yapılabilmektedir. Kliniğimizde üç boyutlu görüntüleme sistemleri eşliğinde laparoskopik radikal prostatektomi uygulanmaktadır. Bu yöntemde kullanılan 3 boyutlu görüntüleme sayesinde robotik cerrahinin etkinliğinin, konvansiyonel laparoskopi maliyetinde elde edilebileceğini göstermeyi amaçladık. Çalışmamızda 2015 yılından başlayarak Fırat Üniversitesi Tıp Fakültesi Üroloji Kliniğinde, 3 boyutlu görüntüleme eşliğinde yapılan laparoskopik radikal prostatektomili hastalar retrospektif olarak taranmıştır. Çalışmaya 69 hasta dâhil edilmiştir. Hastaların elde edilen onkolojik, fonksiyonel, preoperatif, perioperatif ve postoperatif sonuçları literatürde yer alan açık retropubik radikal prostatektomi, iki boyutlu görüntüleme eşliğinde yapılan laparoskopik radikal prostatektomi ve robot yardımlı radikal prostatektomi sonuçlarıyla karşılaştırıldı. Elde edilen verilerimiz literatürle karşılaştırıldığında onkolojik ve fonksiyonel sonuçlarımızın literatür verileriyle paralellik gösterdiği tespit edildi. Hastanede yatış süresi, kanama miktarı, uretral kataterizasyon süresi, cerrahi sınır pozitifliği, PSA progresyonu gibi diğer parametrelerin de RARP literatür verileriyle paralellik gösterdiği sonucuna varılmıştır. Üç boyutlu görüntüleme eşliğinde LRP maliyetinin RARP'ye kıyasla çok daha düşük olması ve RARP'de sağlanamayan dokunma hissinin uyguladığımız üç boyutlu LRP'de elde edilebilmesi lokalize prostat kanseri tedavisinde üç boyutlu görüntüleme eşliğinde LRP'nin üstünlüğünü göstermektedir. The Place Of Three Dimensional Laparoscopy İn Localized Prostate Canser Prostate cancer is the second most common cancer in men. The gold standard treatment for localized prostate cancer is radical prostatectomy. At the present time, radical prostatectomy can be performed with open (retropubic, perineal),laparoscopic and robot-assisted laparoscopic methods. In our clinic, laparoscopic radical prostatectomy is performed with three-dimensional imaging systems. We aimed to show that the effectiveness of robotic surgery can be achieved at the cost of conventional laparoscopy thanks to the 3D imaging used in this method. In our study, patients with a history of laparoscopic radical prostatectomy performed with 3D imaging in the Urology Clinic of Fırat University Faculty of Medicine, starting from 2015, were retrospectively scanned. 69 patients were included in the study. The oncological, functional, preoperative, perioperative and postoperative results of the patients were compared with the results of open retropubic radical prostatectomy, laparoscopic radical prostatectomy performed with two-dimensional imaging, and robot-assisted radical prostatectomy in the literature. When our data were compared with the literature, it was determined that our oncological and functional results were in parallel with the literature data. It was concluded that other parameters such as length of hospital stay, amount of bleeding, duration of urethral catheterization, positive surgical margin, and PSA progression were in parallel with the RARP literature data. The fact that the cost of LRP with three-dimensional imaging is much lower compared to RARP and that the tactile sensation which cannot be achieved in RARP can be obtained in three-dimensional LRP, which we have performed, shows the superiority of LRP with three-dimensional imaging in the treatment of localized prostate cancer.
