dc.description.abstractÖZET Vokal kordlar, fonasyonun sağlıklı bir şekilde sürdürülebilmesi için bütüncül bir histolojik yapı ve dengeli bir ekstrasellüler matriks organizasyonu gerektirir. Vokal kord cerrahileri, travmalar ve çeşitli iatrojenik girişimler sonrasında gelişen skar dokusu; lamina proprianın viskoelastik özelliklerini bozarak mukozal dalga hareketini kısıtlamakta ve kalıcı ses bozukluklarına yol açabilmektedir. Bu nedenle, vokal kord yara iyileşmesini destekleyen ve skar oluşumunu azaltabilecek biyolojik ajanlara yönelik deneysel çalışmalar güncelliğini korumaktadır. Geleneksel tıpta yaygın olarak kullanılan Hypericum perforatum'un (sarı kantaron) yara iyileştirici ve antiinflamatuar etkileri farklı dokularda gösterilmiş olsa da, vokal kord skar iyileşmesi üzerindeki etkileri literatürde daha önce araştırılmamıştır. Bu çalışmanın amacı, deneysel olarak oluşturulmuş vokal kord hasarı sonrası geleneksel yöntemle hazırlanmış Hypericum perforatum ekstresinin histopatolojik iyileşme ve ekstrasellüler matriks yeniden yapılanması üzerindeki etkilerini sıçan modelinde değerlendirmektir. Fırat Üniversitesi Hayvan Deneyleri Etik Kurulu onayı ile 35 adet 8–10 haftalık, 200–250 g ağırlığında erkek Sprague-Dawley sıçan çalışmaya dahil edildi. Endoskopik görüş altında 980 nm dalga boyunda diyot lazer ile sol vokal kordda kontrollü doku hasarı oluşturuldu. Hypericum perforatum ekstraktı, bitkinin çiçekli uçlarının dört hafta boyunca saf zeytinyağı içerisinde güneşte bekletilmesiyle geleneksel olarak hazırlandı. Denekler rastgele gruplara ayrıldı: Grup A (Tedavi verilmeyen grup, n=7), Grup B (HP ekstraktı uygulanan, n=14; B1: tek doz, B2: dört doz) ve Grup C (saf zeytinyağı uygulanan, n=14; C1: tek doz, C2: dört doz). Grup A'nın sağ vokal kordu kontrol grubu olarak kullanıldı. Tedavi uygulamaları, yaralanma sonrası sol vokal kord yüzeyine ekstrakt veya yağ emdirilmiş spongellerle 10 dakika süreyle yapıldı. 21. günde tüm denekler sakrifiye edilerek larenks dokuları toplandı ve %10'luk formalinde sabitlendi. Kesitler Hematoksilen-Eozin, Masson trikom ve Verhoeff-Van Gieson boyaları ile boyandı. Her örnek, epitel rejenerasyonu, inflamasyon, granülasyon dokusu, submukozal fibroblast yoğunluğu, kollajen depolanması ve elastin mevcudiyeti açısından yarı kantitatif olarak derecelendirildi. Verilerin istatistiksel analizinde Kruskal-Wallis testi ve Dunn post-hoc testi kullanıldı (p<0,05). Histopatolojik değerlendirmede, tekrarlayan doz Hypericum perforatum uygulanan grupta (B2) fibroblast yoğunluğu ve kollajen depolanmasının tedavi verilmeyen gruba kıyasla istatistiksel olarak anlamlı şekilde azaldığı, elastin lif organizasyonunun ise istatistiksel olarak anlamlı şekilde daha düzenli olduğu gözlendi. Tek doz Hypericum perforatum uygulanan grupta (B1), kollajen birikiminin tedavi verilmeyen gruba göre anlamlı şekilde azaldığı gözlense de fibroblast yoğunluğu ve elastin depolanmasındaki değişiklikler anlamlı değildi. Epitel rejenerasyonu, enflamasyon ve granülasyon dokusu açısından gruplar arasında belirgin fark izlenmedi. Grup C1 ve C2 ile Grup A arasında ekstrasellüler matriks bileşenleri açısından anlamlı fark saptanmadı. Geleneksel yöntemle hazırlanan Hypericum perforatum yağ ekstraktı, sıçan vokal kord yaralanmasında ekstrasellüler matriksin yeniden yapılanmasını modüle ederek fibroblast proliferasyonunu sınırlamış ve aşırı kollajen birikimini azaltmıştır. Bu etkilerle birlikte elastin lif mimarisinin korunması gözlenmiş olup aşırı fibrotik skar oluşumu sınırlanmış görünmektedir. Bu deneysel çalışma, literatürde ilk kez Hypericum perforatum'un vokal kord skar iyileşmesine etkisini histolojik düzeyde ortaya koyarak alana öncü katkı sağlamaktadır. Bu bulgular, Hypericum perforatum'un vokal kord cerrahileri sonrası skar gelişimini azaltmaya yönelik potansiyel bir destekleyici ajan olabileceğini düşündürmekte olup, klinik uygulamalara geçiş öncesinde ileri deneysel ve klinik çalışmalarla desteklenmesi gerekmektedir. dc.description.abstractABSTRACT EFFECT OF HYPERICUM PERFORATUM ON WOUND HEALING AFTER VOCAL FOLD INJURY Healthy vocal fold function requires an intact histological structure and a well-balanced extracellular matrix (ECM) organization. Vocal fold scarring that develops following surgical procedures, trauma, and various iatrogenic interventions disrupts the viscoelastic properties of the lamina propria, restricts mucosal wave propagation, and leads to persistent voice disorders. Therefore, experimental studies investigating biological agents that may enhance vocal fold wound healing and reduce scar formation remain highly relevant. Although Hypericum perforatum (St. John's wort), which is widely used in traditional medicine, has demonstrated wound-healing and anti-inflammatory effects in various tissues, its effects on vocal fold scar healing have not previously been investigated. The aim of this study was to evaluate the effects of traditionally prepared Hypericum perforatum extract on histopathological healing and extracellular matrix remodeling following experimentally induced vocal fold injury in a rat model. With the approval of the Fırat University Animal Experiments Ethics Committee, a total of 35 male Sprague–Dawley rats aged 8–10 weeks and weighing 200–250 g were included in the study. Under endoscopic visualization, a controlled tissue injury was created on the left vocal fold using a 980-nm diode laser. Hypericum perforatum extract was traditionally prepared by macerating the flowering tops of the plant in pure olive oil under sunlight for four weeks. The animals were randomly assigned to groups as follows: Group A (untreated group, n = 7), Group B (Hypericum perforatum extract, n = 14; B1: single dose, B2: four doses), and Group C (pure olive oil, n = 14; C1: single dose, C2: four doses). The right vocal fold of Group A served as the control. Treatments were applied to the injured left vocal fold using sponges soaked with the extract or olive oil for 10 minutes. On postoperative day 21, all animals were sacrificed, and laryngeal tissues were harvested and fixed in 10% formalin. Tissue sections were stained with Hematoxylin–Eosin, Masson's trichrome, and Verhoeff–Van Gieson stains. Each specimen was semi-quantitatively evaluated for epithelial regeneration, inflammation, granulation tissue, submucosal fibroblast density, collagen deposition, and elastin content. Statistical analysis was performed using the Kruskal–Wallis test followed by Dunn's post-hoc test (p < 0.05). Histopathological evaluation demonstrated that in the repeated-dose Hypericum perforatum group (B2), fibroblast density and collagen deposition were significantly reduced compared with the injury-only group, while elastin fiber organization was significantly more regular. In the single-dose Hypericum perforatum group (B1), collagen deposition was significantly reduced compared with the injury group; however, changes in fibroblast density and elastin deposition were not statistically significant. No significant differences were observed among the groups in terms of epithelial regeneration, inflammation, or granulation tissue formation. No significant difference was detected between Groups C1 and C2 and Group A in terms of extracellular matrix components. Traditionally prepared Hypericum perforatum oil extract modulated extracellular matrix remodeling in rat vocal fold injury by limiting fibroblast proliferation and reducing excessive collagen deposition. Preservation of elastin fiber architecture was also observed, suggesting attenuation of excessive fibrotic scar formation. This experimental study is the first to demonstrate the effects of Hypericum perforatum on vocal fold scar healing at the histological level, thereby providing a pioneering contribution to the literature. These findings suggest that Hypericum perforatum may represent a potential adjunctive agent for reducing scar formation following vocal fold surgery; however, further experimental and clinical studies are required before translation into clinical practice.
