TİBİA PLATO KİRİKLARİNİN CERRAHİ TEDAVİ SONUÇLARİNİN DEĞERLENDİRİLMESİ

Abstract

Vücut ağırlığının büyük bir kısmını taşıyan diz eklemini ilgilendiren tibia üst uç kırıkları, sıklıkla fonksiyonel bozukluklara yol açan ciddi yaralanmalardır. Bu çalışmanın amacı Fırat Üniversitesi Hastanesi Ortopedi ve Travmatoloji kliniğinde Ocak 1999 ? Aralık 2007 tarihleri arasında, tibia plato kırığı tanısıyla cerrahi olarak tedavi edilen olguların uzun dönem fonksiyonel sonuçlarının değerlendirilmesi ve kıyaslanması, olgularımızdaki başarı oranını ve ortaya çıkmış olan komplikasyonları belirlemek, gruplandırılmış olguları kaşılaştırılarak kırık tipinin, yaşın fonksiyonel sonuç ve başarı oranı üzerindeki etkisini ortaya koymaktır.Tibia plato kırığı mevcut olan 17'i kadın 32'si erkek 49 olgu tez kapsamına alındı ve olgular retrospektif olarak incelendi. Uygulanan ameliyatlarda hastalar 3 ayrı grupta incelendi. 1. grup açık redüksiyon internal tespit, 2. grup kapalı redüksiyon perkütan tespit, 3. grup kapalı redüksiyon eksternal fiksasyon idi. Her bir grup Rasmussen klinik ve radyolojik değerlendirme skoruna göre kendi içinde analiz edildi.Klinik değerlendirme kriterine bakıldığında 1. gruptaki 18 olguda % 83.4'lük, 2. gruptaki 15 olguda % 93.4'lük, 3. gruptaki 16 olguda ise % 73.5'lik iyi ve mükemmel sonuçlar elde edilmiştir. Radyolojik değerlendirmeye bakılığında ise sırasıyla 1. grupta % 55.6, 2. grupta % 66.6 ve 3. grupta ise % 68.8 başarılı sonuçlar alınmıştır.Sonuç olarak tibia plato kırıkları eklemi ilgilendiren kırıklar olduğundan, kırığın tipi ve yeri, yer değiştirme ve parçalanmanın derecesi ve eşlik eden kemik ve yumuşak doku hasarları dikkate alınarak uygun cerrahi yöntem seçilmelidir.

The tibial superior end fractures that are associated with the knee joint supporting most of the body weight are severe injuries which may lead to frequent functional disorders. The purpose of this study is to evaluate and compare the long term functional results obtained with cases that were treated in Fırat University Hospital Orthopedics and Traumatology clinic between the dates of January 1999 and December 2007, diagnosed with tibial plateau fractures; to determine the success rate of the cases and the complications occurred; and to introduce the effect of the fracture type and age on the functional result and success rate by comparing the grouped cases.A total of 49 cases, 17 of which are women and the rest is men, were included in the scope of thesis and the cases were examined retrospectively. The patients were examined within 3 groups during the operations applied. 1. Group 1 was open reduction internal fixation, group 2 was closed reduction percutaneous fixation, and the third group was closed reduction external fixation. Each group was analyzed as per Rasmussen clinical and radiological evaluation score.Considering the criterion for clinical evaluation, good and excellent results were obtained for 18 cases in group 1, for 15 cases in group 2 and for 16 cases in group 3 which are 83.4%, 93.4% and 73.5%, respectively. When examining the radiological evaluation, the percentages of successful results for group 1, group 2 and group 3 are 55.6%, 66.6% and 68.8%, respectively.Consequently, as tibial plateau fractures are articular fractures, proper surgical method should be selected by taking into consideration the type and area of fracture, degree of migration and fracture, and the accompanying damages of bone and soft tissue.
