dc.description.abstractNazal septum perforasyonu, bilateral mukoperikondriyum ve/veya mukoperiosteum bütünlüğünün bozulmasına bağlı olarak gelişen, kıkırdak ve/veya kemik dokuda nekroz ile seyreden ve iki nazal pasaj arasında patolojik bağlantı oluşmasına neden olan klinik bir durumdur. Perforasyonun büyüklüğüne ve yerleşimine bağlı olarak burun tıkanıklığı, kabuklanma, epistaksis, burundan ıslık sesi gelmesi ve nazal hava akımında bozulma gibi semptomlara yol açabilmekte, buna bağlı olarak hastaların yaşam kalitesini olumsuz etkileyebilmektedir. Nazal septum perforasyonunun tedavisinde konservatif yaklaşımlar, septal buton uygulamaları ve çeşitli cerrahi teknikler tanımlanmış olmakla birlikte, ideal tedavi yöntemi konusunda günümüzde hâlen kesin bir görüş birliği bulunmamaktadır. Son yıllarda doku iyileşmesini artırmaya ve cerrahi başarıyı yükseltmeye yönelik olarak biyomateryaller ve biyolojik ajanların kullanımı giderek önem kazanmıştır. Plateletten zengin plazma (PRP), içerdiği büyüme faktörleri aracılığıyla yara iyileşmesini destekleyen otolog bir ürün iken, epidermal büyüme faktörü (EGF) epitelizasyonu ve hücresel proliferasyonu artıran önemli bir biyolojik ajandır. Kompozit mesh ise yapısal destek sağlayabilen sentetik bir materyal olmasına rağmen, nazal septum perforasyonu onarımındaki etkinliği sınırlı sayıda çalışmada değerlendirilmiştir. Bu çalışmanın amacı, deneysel olarak oluşturulan nazal septum perforasyonu modelinde Kompozit mesh, PRP ve EGF uygulamalarının perforasyon iyileşmesi üzerindeki etkilerini makroskopik ve histopatolojik olarak değerlendirmektir. Fırat Üniversitesi Hayvan Deneyleri Etik Kurulu onayı alınarak yürütülen bu çalışmaya toplam 20 adet tavşan dahil edildi ve denekler her grupta 5 denek bulunacak şekilde dört gruba ayrıldı. Tüm deneklere nazal septum perforasyonu yapıldıktan sonra Kontrol grubu (n=5) oluşturuldu ve bu gruptaki deneklere septum perforasyonu oluşturulduktan sonra herhangi bir işlem yapılmadı. MESH grubundaki deneklere (n=5) septum perforasyonu oluşturulduktan sonra perforasyon alanına Kompozit mesh yerleştirildi. MESH+ PRP grubundaki deneklere (n=5) oluşturulan perforasyon sonrası Kompozit mesh ile birlikte PRP uygulaması yapıldı. MESH+ EGF grubuna (n=5) oluşturulan perforasyondan sonra Kompozit mesh ile birlikte EGF uygulandı. Çalışma süresi sonunda denekler sakrifiye edilerek nazal septumları rezeke edildi. Çıkarılan dokular %10'luk formalinde sabitlendi. Kesitler Hematoksilen-Eozin, Masson trikom ve Verhoeff-Van Gieson boyaları ile boyandı ve ışık mikroskobunda değerlendirildi. Epitel rejenerasyonu, inflamasyon, fibroblastik aktivite, kollajen birikimi, elastik lif oluşumu, neovaskülarizasyon ve genel yara iyileşmesi parametreleri açısından gruplar karşılaştırıldı. Histopatolojik ve makroskopik değerlendirmelerde, Kompozit mesh uygulanan grupta (MESH Grubu) perforasyon alanında yapısal destek ve epitel bütünlüğünün yeniden sağlanmasına katkı gözlenmekle birlikte, bu etkinin yara iyileşmesinin biyolojik bileşenleri açısından sınırlı kaldığı saptandı. Buna karşılık, Kompozit mesh ile birlikte PRP ve EGF uygulanan gruplarda epitel rejenerasyonu, fibroblast proliferasyonu, kollajen sentezi, elastik lif organizasyonu ve neovaskülarizasyon açısından daha belirgin iyileşme bulguları elde edildi (P<0, 05). Özellikle biyolojik ajanlarla desteklenen gruplarda, perforasyon alanında doku bütünlüğünün yeniden sağlanmasına yönelik histopatolojik göstergelerin daha olumlu olduğu ve yara iyileşmesinin hem hücresel hem de ekstrasellüler matriks düzeyinde daha etkin seyrettiği izlendi. Sonuç olarak, nazal septum perforasyonu onarımında Kompozit mesh uygulamasının tek başına yapısal destek sağladığı, ancak PRP ve EGF ile kombine edildiğinde yara iyileşmesi sürecini daha etkin biçimde modüle ederek histopatolojik iyileşmeyi belirgin ölçüde artırdığı kanaatine varıldı. Bu bulgular, sentetik interpozisyon materyallerinin biyolojik ajanlarla desteklenmesinin nazal septum perforasyonu tedavisinde umut verici bir yaklaşım olabileceğini düşündürmektedir. Bununla birlikte, elde edilen sonuçların klinik uygulamaya aktarılabilmesi için daha geniş örneklemli, uzun süreli izlem içeren ve klinik temelli ileri çalışmalara gereksinim bulunmaktadır.
dc.description.abstractNasal septal perforation is a clinical condition that develops due to disruption of the integrity of the bilateral mucoperichondrium and/or mucoperiosteum, progresses with necrosis in the cartilaginous and/or bony tissue, and causes the formation of a pathological connection between the two nasal passages. Depending on the size and location of the perforation, it may lead to symptoms such as nasal obstruction, crusting, epistaxis, whistling from the nose, and impairment of nasal airflow, thereby negatively affecting patients' quality of life. Although conservative approaches, septal button applications, and various surgical techniques have been described in the treatment of nasal septal perforation, there is still no definite consensus today regarding the ideal treatment method. In recent years, the use of biomaterials and biological agents to increase tissue healing and improve surgical success has gained increasing importance. While platelet-rich plasma (PRP) is an autologous product that supports wound healing through the growth factors it contains, epidermal growth factor (EGF) is an important biological agent that increases epithelialization and cellular proliferation. Although composite mesh is a synthetic material capable of providing structural support, its effectiveness in nasal septal perforation repair has been evaluated in a limited number of studies. The aim of this study was to evaluate the effects of composite mesh, PRP, and EGF applications on perforation healing in an experimentally created nasal septal perforation model macroscopically and histopathologically. This study, conducted with the approval of the Fırat University Animal Experimentation Ethics Committee, included a total of 20 rabbits, which were divided into four groups, with five subjects in each group. After nasal septum perforation was performed on all subjects, a Control group (n=5) was established; no further procedures were performed on the subjects in this group following the creation of the septum perforation. In the MESH group (n=5), a composite mesh was placed at the perforation site after the septum perforation was created. In the MESH+ PRP group (n=5), PRP was applied along with the composite mesh following the perforation. In the MESH+ EGF group (n=5), EGF was applied along with the composite mesh following the perforation. At the end of the study period, the subjects were sacrificed, and their nasal septa were resected. The excised tissues were fixed in 10% formalin. Sections were stained with Hematoxylin-Eosin, Masson's trichrome, and Verhoeff-Van Gieson stains and evaluated under a light microscope. The groups were compared in terms of epithelial regeneration, inflammation, fibroblastic activity, collagen deposition, elastic fiber formation, neovascularization, and general wound healing parameters. In histopathological and macroscopic evaluations, while the group treated with composite mesh (MESH Group) demonstrated contributions to the restoration of structural support and epithelial integrity in the perforation site, it was determined that this effect remained limited in terms of the biological components of wound healing. In contrast, in the groups treated with composite mesh combined with PRP and EGF, more pronounced improvements were observed in terms of epithelial regeneration, fibroblast proliferation, collagen synthesis, elastic fiber organization, and neovascularization (P < 0.05). In particular, in the groups supported by biological agents, histopathological indicators suggesting the restoration of tissue integrity in the perforation area were more favorable, and wound healing was observed to proceed more effectively at both the cellular and extracellular matrix levels. As a result, it was concluded that composite mesh application alone provided structural support in nasal septal perforation repair, but when combined with PRP and EGF, it modulated the wound healing process more effectively and significantly increased histopathological healing. These findings suggest that supporting synthetic interposition materials with biological agents may be a promising approach in the treatment of nasal septal perforation. However, in order for the obtained results to be translated into clinical practice, there is a need for further studies with larger sample sizes, including long-term follow-up and based on clinical design.
