TAVŞANLARDA DENEYSEL OLARAK OLUŞTURULAN İNTRA ABDOMİNAL ADEZYONLARIN ÖNLENMESİNDE; POLYETHYLENE GLYCOL VE DEXTRAN'IN KARŞILAŞTIRMALI ARAŞTIRILMASI

Abstract

Bu çalışmada; tavşanlarda deneysel olarak oluşturulan intraabdominal adezyonların önlenmesinde ve lyzisten sonra tekrar şekillenen adezyonların geri letilmesinde veya tamamen yok edilmesinde % 5 polyethylene glycol (PEG)4000 ve % 32 dextran 70 'in karşılaştırılması yapıldı. Araştırmamız iki gruptan ve her grupta kendi içinde 3 altgruptan oluştu. Grup l'de; PEG 1, DEXTRAN 1 ve KONTROL 1 a ltgrupl arında test solüsyonlarının postoperatif adezyonların önlenmesindeki etkileri araştırıldı. Grup 2 'de; PEG 2. DEXTRAN 2 ve KONTROL 2 a ltgrupl arında adezyon oluşturulduktan sonra, test solüsyonlarının şekillenmiş olan adezyonları geriletme veya tamamen yok etmelerindeki etkilerine bakıldı. Ayrıca defekt oluşturulan bölge ve adezyonlu bölgelerin histopatolojik muayeneleri yapılarak adezyon dereceleri mikro olarakta değerlendirildi. Karaciğer, dalak ve böbreğin histo patolojik muayeneleri yapılarak test solüsyonlarının etkileri araştırıldı. Ensizyon çizgisinin histopatolojik muayenesi yapılarak test solüsyonlarının yara iyileşmesi üzerine olan etkisi değerlendirildi. Grup l'de; % 5 PEG 4000 adezyonların önlenmesinde. % 32 Dextran 70 ve Kontol a ltgrupl arına göre anlamlı derecede ya rarlı olduğu saptandı (P <<: 0.05). % 32 Dextran 70'in ise Kontrol alt grubuna göre anlamlı derecede yararlı olduğu görüldü (P s< 0.05). Grup 2 'de; % 5 PEG 4000 'nin lyzisten sonra tekrar şekil lenen adezyonların geriletilmesi veya tamamen yok edilmesin de istatiksel olarak önemli derecede yararlı olduğu anlaşıldı(P^ 0.01). % 32 Dextran 70 'in adezyonların geri let ilmesi veya orta dan kaldırılmasında anlamlı derecede adezyonları artırdığı görüldü (P ^ 0.01). Kontrol altgrubunda ise mevcut adezyonların anlamlı de recede daha da arttığı saptandı (P ^ 0.01). Histopatolojik muayenelerde; test solüsyonlarının hiç bir toksik etkisinin ve yara iyileşmesi üzerine zararlı etkilerinin olmadığı anlaşıldı. Adezyonlu bölgenin ve defektli alanın mikro olarak degerledirilmesi sonucu, f ibroblastların Derece 2'den Derece 4'e doğru yoğunluk kazandığı gözlendi. Sonuç olarak; % 5 PEG 4000; gerek birinci operasyonlar da ve gerekse ikinci operasyonlardan sonra koruyucu olarak kullanılabildiği gibi, operasyon yoluyla kalın band adezyonların elle çekerek veya keserek ayırma işleminden sonra tekrar adezyon oluşmaması için kullanılmasının yararlı olacağı kanısındayız.In this study, the effects of 32% Dextran 70 and 5% PEG 4000 on intraabdominal adhesions were compared. Intraabdominal adhesions were formed experimentally. The drugs were tested to prevent, to regress or to eliminate adhesions. Experiments were carried on two test groups. In the first group, preventive effects of PEG 1 and Dextran 1 on postope rative adhesions were investigated. In the second group PEG 2 and Dextran 2 were used to regress or to eliminate preformed adhesions. In both test groups control animals did not receive any drug solutions. Additionally adhesive and defective tissues were examin ed histopathological ly. The effects of test solutions on the liver, the spleen and kidneys were investigated by histopatho logical examination. In another experiment, the influence, of test solutions on wound healing was investigated by macroscopic examination of the line of incision. The results, of this study show that in group 1 compared 32% Dextran 70 application to 5% PEG 4000 was more effective for prevention of post operative adhesions (P ^ 0.05). However in 32% Dextran 70 group animals the adhesions were less prevelant than in animals of control group (P ^ 0.05). 5% PEG 4000 (Group 2) was found useful 1 in regression or elimination of adhesions.reformed after lysis, in statis tically (P ^ 0.01). It was ascertained that adhesions had increased in 32%88 Dextran 70 and control groups (P ^ 0.01). Histopathological examinations indicated that the test solutions did not have any toxic effects on tissues and vis ceral organs. Futhermore test solutionsn dit not have any adverse affect on wound healing. An increase fibroblast population from degree 2 to deg ree 4 in adhesive and defective tissues were noted in micros copic examination. In conclusion, this study shows that 5% PEG 4000 appli cation might be considered for prevention of post operative adhesions. 5 % PEG 4000 solution seems to be effective in both single and multiple operations. 5% PEG 4000 application not only prevented adhesions but also regressed and elimina ted preformed adhesions/ Thus, it is advisible to use topical application of these drugs in cases where post operative adhesion pose a signifi cant complication.
