PAPİLLER TİROİD KARSİNOMUNDA BRAF V600E MUTASYONU İLE TROP2 VE PD-L1 EKSPRESYONU ARASINDAKİ İLİŞKİ

Abstract

Tiroid kanserleri içerisinde en sık görülen papiller tiroid karsinomudur (PTK). PTK varyantlarının histopatolojik özellikleri benzerlik gösterse de prognostik açıdan farklı davranışlar sergileyebilirler. Varyantlardaki prognoz farklılığına yönelik yapılan araştırmalarda mutasyon varlığı ve bazı moleküllerin ekspresyonunda artış bulundu. Son yıllarda malign tanısı alan olgu sayısındaki artış, moleküler yöntemlerin ilerlemesi, hedefe yönelik tedavilerin gelişmesi, erken tanı, prognoz ile hasta takibinde yol gösterecek belirleyicilere yönelik çalışmaların artmasına neden olmuştur. Çalışmamızda klasik varyant ve folliküler varyant papiller tiroid karsinomu tanısı alan olgularda BRAF V600E mutasyonu, trop2, PD-L1, galektin-3, HBME-1 ve CK19 ekspresyonunun prognostik parametreler ve histomorfolojik bulgularla ilişkisini immunohistokimyasal yöntemlerle araştırmayı amaçladık. Çalışmamızda 2010-2018 yılları arasında PTK tanılı 40 KVPTK, 40 FVPTK olgusuna ait parafin bloklara immunohistokimyasal olarak BRAF V600E, trop2, PD-L1, galektin-3, HBME-1, CK19 uyguladık. Sonuçları yaş, cinsiyet, histolojik tip, ekstratiroidal yayılım, stromal kalsifikasyon, psammom, tümör boyutu, multifokalitesi, tümör evresi, nüks/uzak metastaz, sağ kalım ile karşılaştırdık. Çalışmamızda KVPTK hastalarının %77.5, %95, %80, %97.5, %92.5, %92.5'i sırasıyla BRAF V600E, PD-L1, trop2, HBME-1, galektin-3 ve CK19 ile pozitifti. FVPTK hastalarının ise %82.5, %97.5, %35, %82.5, %45, %72.5'i sırasıyla BRAF V600E, PD-L1, trop2, HBME-1, galektin-3 ve CK19 ile pozitifti. Trop2 ve galektin-3 ekspresyonu ile histolojik tip arasında anlamlı ilişki saptandı. FVPTK ile yalnızca T evresi arasında anlamlı ilişki görüldü. Her iki varyant ile diğer parametreler arasında anlamlı ilişki saptanmadı. Sonuç olarak, BRAF V600E mutasyonunun kötü prognozla ilişkili olmadığını, PD-L1 ile trop2'nin yalnızca nüks/uzak metastaz prognostik parametresiyle ilişkili olduğunu saptadık. HBME-1, galektin-3 ve CK19 ekspresyonunun ise kötü prognozla ilişkili olduğunu tespit ettik. Anahtar Kelimeler: Papiller Tiroid Karsinomu, BRAF V600E, trop2, PD-L1

RELATIONSHIP BETWEEN BRAF V600E MUTATION WITH TROP2 AND PD-L1 EXPRESSION IN PAPILLARY THYROID CARCINOMA Papillary thyroid carcinoma (PTC) is the most common thyroid cancer. Although the histopathological features of PTC variants are similar, they may exhibit different prognostically different behaviors. Studies on the difference in prognosis in variants found the presence of mutations and an increase in the expression of some molecules. In recent years, the increase in the number of cases diagnosed with malignancy, the advancement of molecular methods, the devolopment of targeted therapies have led to an increase in studies on determinants that will guide early diagnosis, prognosis and patient follow up. In our study, we aimed to investigate the relationship between BRAF V600E mutation, trop2, PD-L1, galectin-3, HBME-1 and CK19 expression with prognostic parameters and histomorphological findings by immunohistochemical methods in patients diagnosed with classic variant and follicular variant PTC. In our study, we applied immunohistochemically BRAF V600E, trop2, PD-L1, galectin-3, HBME-1, CK19 to paraffin blocks of 40 CVPTC and 40 FVPTC cases between 2010 and 2018. We compared the results with age, gender, histological type, extrathyroidal extension, stromal calsification, psammoma, tumor size, multifocality, T stage, recurrence/distant metastasis, survival. In our study, 77.5%, 95%, 80%, 97.5%, 92.5%, 92.5% of CVPTC patients were positive with BRAF V600E, PD-L1, trop2, HBME-1, galectin-3 and CK19, respectively. 82.5%, 97.5%, 35%, 82.5%, 45%, 72.5% of FVPTC patients were positive with BRAF V600E, PD-L1, trop2, HBME-1, galectin-3 and CK19, respectively. Significant relotionship was found between trop2 and galectin-3 expression and histological type. Significant relotionship was seen only between FVPTC and T stage. There was no significant relationship between both variants and other parameters. As a result, we found that the BRAF V600E mutation was not associated with a poor prognosis, and that PD-L1 and trop2 were only associated with the recurrence/distant metastasis prognostic parameter. We found that HBME-1, galectin-3 and CK19 expression was associated with poor prognosis.
