The Role And Importance of Mean Platellet Volume in Diagnosis of Subclinic Intraamniotic Infection And in Follow Up Premature Rupture of Membranes

Abstract

Aim: This study was carried out in order to determine the role and importance of MPV for diagnosis and follow up of subclinical infections in PPROM and to compare the values of ProCT, CRP and WBC. Methods: The patient group consists of 38 patients diagnosed for PPROM and control group consists of 43 healthy pregnant women between 24 and 34 gestational weeks. WBC, ProCT, CRP and MPV values of patients at diagnosis, delivery and values of women in control groupwere assessed and compared. The placental pathology reports of patient group were examined. WBC, ProCT, CRP and MPV values of those having chorioamnionitis were compared. Results: In study group, the WBC 17.01 ± 4.82 , CRP 1.15 ± 0.74 , and ProCT 0.08 ± 0.05 values measured before the delivery were significantly higher than those measured at the moment of diagnosis. Of the study group, the WBC 11.97 ± 2.59 and CRP 0.48 ± 0.29 values measured at the moment of diagnosis were significantly higher than those of control group. Prenatal WBC 17.01 ± 4.82 , CRP 1.15 ± 0.74 and ProCT 0.08 ± 0.05 values of patients were statistically significantly higher than those of control group. CRP 1.73±0.67 values of patients with chorioamnionitis were higher than those of patients having no chorioamnionitis. There was no significant relationship between MPV values of the groups. Conclusion: WBC, CRP and ProCT can be interpreted to be capable of diagnostically predicting the intrauterine infection. No statistically significant relationship was found between MPV, diagnosis and sub-clinic infection in PROM.Amaç: Bu çalışma OTH’nin, preterm EMR‘deki subklinik infeksiyon tanısı ve takibindeki yeri ve öneminin araştırılması, aynı zamanda ProCT, CRP ve BK değerleri ile karşılaştırılması amacıyla yapılmıştır. Metod: Vaka grubu 24-34. gebelik haftalarında preterm erken membran rüptürü tanısı alan 38 hastadan, kontrol grubu ise 24-34. gebelik haftalarında herhangi bir ek hastalığı olmayan 43 sağlıklı gebeden oluşmaktadır. Hastaların tanı anındaki ve doğum öncesindeki BK, ProCT, CRP ve OTH değerleri ile kontrol grubundaki hastaların herhangi bir zamanda alınmış olan BK, ProCT, CRP ve OTH değerleri kaydedilerek birbirleriyle karşılaştırılarak analiz edilmiştir. Ayrıca vaka grubunun plasental patoloji raporları incelenmiş, koryoamniyoniti olan ve olmayan olguların BK, CRP, ProCT ve OTH değerleri karşılaştırılmıştır. Bulgular: Vaka grubunun doğum öncesi ölçülen BK 17,01 ± 4,82 , CRP 1,15 ± 0,74 , ProCT 0,08 ± 0,05 değerleri tanı anında ölçülen değerlere göre anlamlı olarak yüksekti. Vaka grubunda tanı anında ölçülen BK 11,97 ± 2,59 ve CRP 0,48 ± 0,29 değerleri kontrol grubuna göre anlamlı olarak yüksekti. Hastaların doğum öncesi BK 17,01 ± 4,82 , CRP 1,15 ± 0,74 ve ProCT 0,08 ± 0,05 değerleri kontrol grubuna göre anlamlı olarak yüksekti. Ayrıca vaka grubunda koryoamniyoniti olanların CRP 1,73±0,67 değerleri olmayanlara göre anlamlı olarak yüksek bulundu. OTH değerleri vaka ve kontrol gruplarında karşılaştırıldığında aralarında anlamlı bir ilişki bulunmadı. Sonuç: BK, CRP ve ProCT’nin intrauterin infeksiyonu, tanısal olarak öngörebileceği yönünde değerlendirilebilir. OTH ile preterm EMR’deki subklinik infeksiyonun tanısı ve takibi arasında anlamlı bir ilişki bulunamamıştır.
