ACİL SERVİSTE AKUT KORONER SENDROM TANISI ALAN HASTALARDA PLAZMA ELABELA, ASPROSİN'İN DİAGNOSTİK DEĞERİ

Abstract

Çalışmamız Fırat Üniversitesi Tıp Fakültesi Acil Tıp Anabilim Dalı kliniğinde Akut Koroner Sendrom (AKS) (ST elevasyonlu miyokard infarktüsü (STMI), ST segment yüksekliği olmayan miyokard infarktüsü (NSTMI), anstabil anjina pektoris (USAP)) tanısı konarak kardiyoloji servisine yatışı yapılan 112 hasta dahil edildi. Kontrol grubu herhangi bir hastalığı olmayan gönüllü kişilerden (58 kişi) oluştu. Çalışmadaki amacımız, acil serviste akut koroner sendrom (AKS) tanısı konulan hastalarda, elabela ve asprosinin diagnostik değerini saptamaktı. Çalışma Fırat Üniversitesi Tıp Fakültesi acil servisine göğüs ağrısıyla başvuran 93 MI (55 STMI, 38 NSTMI), 19 USAP tanısı konulan hastalardan oluşturuldu. Çalışma verilerinin değerlendirilmesinde; asprosin için AKS tanılı ve sağlıklı kontrol grupları arasında anlamlı fark mevcuttu. Çalışma verilerine göre yapılan ROC analizinde elabela için cutoff değeri ?1110,303 iken; sensivite %79, spesifite %28 bulundu (AUC: 0,502, %95, Confidence interval: 0,42-0,57, p=0,9659) ve yine çalışma verilerine göre yapılan ROC analizinde asprosin için cutoff değeri ?67,298 iken; sensivite %82, spesifite %37 bulundu. (AUC: 0,610, %95, Confidence interval: 0,53-0,68, p=0,0191) Sonuç olarak; asprosinin diagnostik değeri elabelaya göre daha yüksektir. Acil servise göğüs ağrısıyla başvuran hastalarda asprosin düzeylerinin ayırıcı tanıda ve erken biyomarker olarak kullanılabilmesi için daha fazla çalışmaya ihtiyaç vardır. THE DIAGNOSTIC VALUE OF ELABELA AND ASPROSIN IN PATIENTS WITH ACUTE CORONARY SYNDROME IN THE EMERGENCY DEPARTMENT The purpose of this study is to determine the diagnostic value of elabela and asprosin in patients with acute coronary syndrome (ACS). The study includes 112 ACS (STEMI, NSTEMI and UA) patients that presented to the emergency department with chest pain and were hospitalized in the cardiology clinic of Fırat University Hospital. The control group consisted of 58 healthy volunteers. Among the 112 ACS patients, 93 presented to the hospital with myocardial infarction (MI) (55 STEMI and 38 NSTEMI) and 19 with unstable angina (UA). There was a significant difference between the levels of asprosin in experimental and control groups. ROC analysis revealed the cut-off value of elabela to be ?1110.303 (sensitivity, 79%; specificity, 28%; AUC = 0.502, 95% confidence interval = 0.42-0.57; p = 0.9659). The diagnostic threshold of asprosin was ?67.298 (sensitivity, 82%; specificity, 37%; AUC = 0.610; 95% confidence interval = 0.53-0.68, p = 0.0191) We conclude that asprosin has a higher diagnostic value than elabela. We recommend further studies to be able to utilize asprosin levels for the differential and early diagnosis of patients that present to the emergency department with chest pain.
