DİETİLNİTRÖZAMİN İLE OLUŞTURULMUŞ HEPATİK FİBROZİS MODELİNDE ENGİNAR YAPRAĞI VE MARESİN-1'İN POTANSİYEL KORUYUCU ETKİLERİ

Hepatik fibrozis, kronik karaciğer hastalıklarının ilerleyişinde kritik bir aşama olup siroz ve hepatoselüler karsinom gibi ciddi komplikasyonlara yol açabilmektedir. Bundan dolayı etkili tedavi yöntemlerinin geliştirilmesi büyük önem taşımaktadır. Mevcut tedavi seçeneklerinin sınırlı olması, alternatif yaklaşımlara olan ihtiyacı arttırmaktadır. Doğal bileşenlerin antifibrotik etkileri üzerine yapılan çalışmalar, yeni tedavi stratejileri geliştirilmesine katkı sağlayabilir. Bu çalışmada, deneysel hepatik fibrozis modeli oluşturulan sıçanlarda Enginar yaprağı ve Maresin-1'in koruyucu etkilerinin araştırılması amaçlanmıştır. Çalışmada sıçanlar 5 gruba ayrıldı (Kontrol grubu, DEN grubu, DEN+Enginar grubu, DEN+Maresin grubu ve DEN+Enginar+Maresin grubu) ve çalışma 4 hafta sürdü. Kontrol grubuna haftada 2 kez 0,5 ml SF (Serum fizyolojik) intraperitoneal (i.p.) yolla, haftada 2 kez de 0,5 ml distile su oral gavaj yoluyla uygulandı. Kontrol grubu dışındaki bütün gruplara haftada bir kez DEN (70 mg/kg, SF içinde çözülerek) i.p. yolla uygulandı. Bunun yanında DEN grubuna haftada bir kez 0,5 ml SF i.p. yolla, haftada iki kez de 0,5 ml distile su oral gavaj yoluyla uygulandı. DEN+Enginar grubuna haftada bir kez 0,5 ml SF i.p. yolla, haftada iki kez de enginar yaprak ekstraktı (1,5 gr/kg, distile su içinde çözülerek) oral gavaj yoluyla uygulandı. DEN+Maresin grubuna haftada iki kez Maresin-1 (4 ng/gr, SF içinde çözülerek) intraperitoneal yolla, haftada iki kez de 0,5 ml distile su oral gavaj yoluyla uygulandı. DEN+Enginar+Maresin grubuna ise haftada iki kez Maresin-1 (4 ng/gr, SF içinde çözülerek) intraperitoneal yolla, haftada iki kez de enginar yaprak ekstraktı (1,5 gr/kg, distile su içinde çözülerek) oral gavaj yoluyla uygulandı. Uygulamalar sonunda sıçanlar dekapite edilerek alınan kanda ve karaciğer dokusunda, biyokimyasal ve histopatolojik analizler yapıldı. Kontrollerle karşılaştırıldığında, DEN uygulanan fibrozis gruplarında; WNT-5a, B-Katenin, MDA ve GPC-3 düzeylerinde artış saptandı. Tedavi gruplarında ise DEN grubuyla karşılaştırıldığında bu dört parametrenin tamamında ise düşüş saptandı. sKla ve Glutatyon düzeylerinde ise kontrol grubuna kıyasla fibrozis gruplarında azalma saptanmıştır. Tedavi gruplarında ise DEN grubuna kıyasla bu iki parametrede artış tespit edildi. Bu etkiler daha çok Enginar+Maresin gruplarında anlamlı bulunmuştur (p<0,05). Karaciğer dokuları üzerinde yapılan histopatolojik incelemede kontrol grubunda normal histolojik yapı gözlemlendi. DEN grubunda ise inflamasyon, fibrozis ve kupffer hücrelerinde artışın yanı sıra minimal nekroz tespit edilmiştir. Tedavi grupları incelendiğinde ise bu bulgularda düzelme olduğu, özellikle de Enginar+Maresin grubunda hafif inflamasyon dışında herhangi bir fibrozis veya nekroz bulgusuna rastlanmadığı tespit edilmiştir. Bu bulgular sonucunda, DEN modelinin oksidatif stres, inflamasyon ve fibrojenik yanıtları tetikleyerek karaciğer hasarına neden olduğu görülmüştür. Enginar ve Maresin-1 tedavilerinin, özellikle birlikte kullanıldıklarında, oksidatif stresi azaltma, anti-enflamatuar etkiler gösterme, anti-fibrotik özelliklere sahip olma ve biyobelirteç düzeylerini düzenleme açısından etkili olabileceği tespit edilmiştir. Bu da, Enginar+Maresin-1 kombinasyonunun karaciğeri koruyucu bir potansiyel taşıdığını ve temelinde fibrozis ile başlayan karaciğer hastalıklarının tedavisinde umut verici bir yaklaşım sunabileceğini göstermektedir.
Hepatic fibrosis is a critical stage in the progression of chronic liver diseases and can lead to serious complications such as cirrhosis and hepatocellular carcinoma. Therefore, the development of effective treatment methods is of great importance. The limited number of available treatment options increases the need for alternative approaches. Studies on the antifibrotic effects of natural components may contribute to the development of new treatment strategies. The aim of this study was to investigate the protective effects of Artichoke leaf and Maresin-1 in rats with an experimental hepatic fibrosis model. The rats were divided into 5 groups in the study (Control group, DEN group, DEN+Artichoke group, DEN+Maresin group and DEN+Artichoke+Maresin group) and the study lasted for 4 weeks. The control group was administered 0.5 ml of SF (physiological serum) intraperitoneally (i.p.) twice a week and 0.5 ml of distilled water orally gavage twice a week. All groups except the control group were administered DEN (70 mg/kg, dissolved in SF) once a week by i.p. route. In addition, 0.5 ml SF was administered i.p. once a week to the DEN group, and 0.5 ml distilled water was administered orally twice a week. The DEN+Artichoke group was administered 0.5 ml SF i.p. once a week, and artichoke leaf extract (1.5 gr/kg, dissolved in distilled water) was administered orally twice a week. The DEN+Maresin group was administered Maresin-1 (4 ng/gr, dissolved in SF) intraperitoneally twice a week, and 0.5 ml distilled water was administered orally twice a week. In the DEN+Artichoke+Maresin group, Maresin-1 (4 ng/gr, dissolved in SF) was administered intraperitoneally twice a week, and artichoke leaf extract (1.5 gr/kg, dissolved in distilled water) was administered orally twice a week. At the end of the applications, the rats were decapitated and biochemical and histopathological analyses were performed on the blood and liver tissue taken. When compared to the controls, an increase was detected in WNT-5a, B-Catenin, MDA and GPC-3 levels in the DEN-applied fibrosis groups. In the treatment groups, a decrease was detected in all four of these parameters when compared to the DEN group. A decrease was detected in sKla and Glutathione levels in the fibrosis groups compared to the control group. In the treatment groups, an increase was detected in these two parameters compared to the DEN group. These effects were found to be more significant in the Artichoke+Maresin groups. In the histopathological examination of the liver tissues, a normal histological structure was observed in the control group. In the DEN group, inflammation, fibrosis and an increase in Kupffer cells as well as minimal necrosis were detected. When the treatment groups were examined, it was determined that there was an improvement in these findings, and especially in the Artichoke + Maresin group, no fibrosis or necrosis findings were observed except for mild inflammation. As a result of these findings, it was observed that the DEN model caused liver damage by triggering oxidative stress, inflammation and fibrogenic responses. It was determined that Artichoke and Maresin-1 treatments, especially when used together, could be effective in reducing oxidative stress, exhibiting anti-inflammatory effects, having anti-fibrotic properties and regulating biomarker levels. This shows that the Artichoke + Maresin-1 combination has a liver-protective potential and may offer a promising approach in the treatment of liver diseases that begin with fibrosis.
