Anatomic Vascular Variations In Sub-Inguinal Varicocelectomy

Abstract

Objective: Varicocele is the most correctable pathology in male infertility. Especially microsurgical varicocelectomy is a technique applied recently in the past few years. This study evaluates the testicular vascular structures in the sub-inguinal zone in patients whom undergone subinguinal varicocelectomy. Materials and Method: A total of 65 patients who underwent microsurgical sub-inguinal varicoselectomy and whom were contacted data were retrospectively assessed according to number and adjuvant conditions of per-operative artery and vein.Results: Per-operative single artery in 32% (21/65) patients, double arteries in 46% (30/65) patients and three or more arteries in 15% (10/65) patients was determined. The mean number of veins connected during the operation was found 9,76±0,96 (3- 14). The artery vein neighborhood was determined as a vein package in 91% (59/65) patients. However an artery was determinedat a single vein neighborhood in 3% (2/65) of the patients where the artery was separated from the vein package. Arterial structures were not determined clearly in 6% (4/65) of the cases.Conclusion: Microsurgical sub-inguinal varicocelectomy is an effective method in the treatment of varicocele. However, the branching ratios of arterial and venous structures in this region are higher according to the inguinal and high inguinal regions. Therefore it is very important to carry out a better dissection as it may lead to relapses in unconnected veins and may negatively effect the connection of functions of the testicular arteries.Amaç: Varikosel erkek infertilitesinin en sık düzeltilebilir patolojisidir. Özellikle mikrocerrahi varikoselektomi son yıllarda yaygın olarak uygulanmaya başlanan bir yöntemdir. Bu çalışmada subinguinal varikoselektomi yapılan hastalardaki subinguinal bölge testiküler vasküler yapıları değerlendirildi.Gereç ve Yöntem: Kliniğimizde mikrocerrahi subinguinal varikoselektomi uygulanan ve verilerine ulaşılan toplam 65 hastanın peroparatif arter, ven sayı ve komşulukları retrospektif olarak değerlendirildi.Bulgular: Peroperatif olarak hastaların sırasıyla %32 (21/65)'sinde tek arter, %46 (30/65)'sında çift arter, %15 (10/65)'inde üç veya daha fazla sayıda arter belirlendi. Operasyon sırasında bağlanan ortalama ven sayısı 9,76±0,96 (3-14) olarak bulundu. Arter ven komşuluğu % 91(59/65) hastada ven paketi şeklinde saptandı. Arterin ven paketinden ayrı saptandığı %3 (2/65) hastada, arter tek ven komşuluğunda belirlendi. Olguların %6 (4/65)'sında ise arteriyel yapılar net olarak belirlenemedi.Sonuç: Mikrocerrahi subinguinal varikoselektomi, varikosel tedavisinde etkin bir yöntemdir. Ancak bu bölgede arteryel ve venöz yapıların dallanma oranları inguinal ve yüksek inguinal bölgeye göre daha fazladır. Bu nedenle bağlanacak testiküler arterlerin testis fonksiyonunu olumsuz yönde etkileyebileceği, bağlanmayan venlerinde nükse sebep olabileceği dikkate alınarak daha iyi bir diseksiyon yapılması gerekmektedir.
