AŞİL TENDONU TAMİRİ YAPILAN TAVŞANLARDA LOKAL PRP VE OZON UYGULAMASININ TENDON İYİLEŞMESİ ÜZERİNE ETKİLERİ

Abstract

Bu çalışmada, aşil tendon rüptürü tedavisinde PRP ve ozon tedavisinin tendon iyileşmesine etkileri değerlendirilmiştir. Her grupta 8 tavşan olacak şekilde 3 grup oluşturuldu: 1. Grup Tamir (T), 2. Grup Tamir + PRP (TP), 3. Grup Tamir+Ozon (TO). Aşil tendonuna keskin disseksiyonla akut yırtık modeli oluşturuldu. Daha sonra modifiye Kessler tekniği ile tendon tamiri yapıldı. Tendon tamir bölgesine, TP grubunda PRP, TO grubunda ise toplam 4 hafta aşil tendonuna O2O3 gaz karışımı enjekte edildi. Çalışma postoperatif 6. haftada sonlandırıldı. Mevcut deney grupları histolojik ve biyomekanik çalışmaya alındı. Histolojik analizde iyileşmenin en yüksek düzeyde ozon tedavisinde olduğu, PRP tedavisinin de kontrol grubuna göre iyileşme skorlarının belirgin şekilde arttığı gözlemlendi. Yapılan biyomekanik analizde ise çekme testi uygulandı. TO (tamir+ozon) grubunun maksimum dayanımının en yüksek düzeyde olduğu, bunu TP (tamir+PRP) grubunun izlediği, en düşük düzeylerin histolojik analize uyumlu şekilde T (tamir) grubunda olduğu tespit edildi. Yapılan istatistiksel analizde de TO(tamir+ozon) grubunun T (tamir) grubuna göre istatistiksel olarak anlamlı bir artış kaydettiği görüldü (p<0.05). Maksimum uzama değerlerinde üç gruptaki veriler arasında istatistiksel olarak anlamlı bir farklılık tespit edilmedi. Sonuç olarak yapılan histolojik ve biyomekanik analizler ozon tedavisinin aşil tendonu iyileşmesi üzerine etkili olduğunu göstermektedir. PRP tedavisinin histolojik değerlendirmede iyileşmeyi arttırdığı ancak biyomekanik analizde istatistiksel olarak kontrol grubuna göre mukavemeti arttırmadığı görüldü.

In this study, the effects of PRP and ozone therapy on tendon healing in the treatment of Achilles tendon rupture were evaluated. Three groups were formed with 8 rabbits in each group: Group 1 Repair (R), Group 2 Repair + PRP (RP), Group 3 Repair + Ozone (RO). An acute tear model was created by sharp dissection into the Achilles tendon. Then, tendon repair was performed using the modified Kessler technique. O2O3 gas mixture was injected into the Achilles tendon for a total of 4 weeks in the RP group and PRP in the RP group. The study was terminated in the 6th postoperative week. These experimental groups were included in histological and biomechanical study. In histological analysis, it was observed that the improvement was at the highest level in the ozone treatment, and the improvement scores of PRP treatment significantly increased compared to the control group. Tensile test was applied in the biomechanical analysis. In the statistical analysis, it was seen that the RO (repair + ozone) group had a statistically significant increase compared to the R (repair) group. (p<0.05) No statistically significant difference was found between the data in the three groups in the maximum elongation values. As a result, histological and biomechanical analyzes show that ozone therapy is effective on the healing of the Achilles tendon. It was seen that PRP treatment increased the improvement in histological evaluation, but did not statistically increase the strength compared to the control group in biomechanical analysis.
