SOMATİZASYON BOZUKLUĞU OLAN HASTALARDA ORBİTOFRONTAL KORTEKS HACİMLERİ VE KLİNİK DEĞİŞKENLERLE İLŞKİSİ

Abstract

Somatizasyon Bozukluğu (SB) sık görülen ruhsal bozukluklardan olup, halen bu hastalıkların nedenleri hakkında aydınlıtılmamış noktalar bulunmaktadır. Son yıllarda yapılan çok az sayıdaki nöro görüntüleme çalışmaları hastalığın etyolojisiyle beyindeki bazı bölgelerin ilintili olabileceğini göstermiştir. Bu nedenle çalışmamızda somatizasyon bozukluğunun etyopatogenezi daha iyi anlayabilmek adına, hastalarda OFC'teki morfometrik değişiklikleri araştırmak amaçlanıldı.Çalışma süresince Fırat Üniversitesi Fırat Tıp Merkezi Psikiyatri Kliniği ve Polikliniği'ne başvuran ve yatarak ya da ayaktan tedavi gören, DSM-IV tanı ölçütlerine göre SB tanısı almış ve çalışma ölçütlerine uyan 20 hasta ile 20 sağlıklı kontrol grubu çalışmaya dahil edilmiştir. Hastalara sosyodemografik ve klinik bilgi formu, Hamilton depresyon derecelendirme ölçeği, Hamilton anksiyete ölçeği ve SCID-I uygulanmıştır.Hasta ve kontrol grubunda MRG kullanılarak total beyin, gri madde, beyaz madde ve OFC bölümlerinin volümetrik ölçümleri değerlendirildi.SB'de orbitofrontal korteks hacmi kontrollerden anlamlı olarak küçük bulundu.OFC'in hem hastalığın patofizyolojisi hem de klinik seyrinde önemli ilişkisinin olabileceğini söylenebilir. İleride yapılacak çalışmaların desteklenmesi bizimde yaptığımız görüntüleme yöntemlerininde ilave edilmesiyle daha önemli ve verimli sonuçlara ulaşılabilir.

Somatization disorder (SD) is a common mental disorders, there are stil spots illuminated about the causes of this disease. Recently, in neuroimaging studies, some abnormalities that may be related to thesigns and symptoms of the disease have been shown. In our study we were aimed to investigate the morphometric alterations of orbitofrontal cortex in the somatization patients to understand the pathophsiology of the disorder.Somatization disorder (SD) is a common mental disorders, there are stil spots illuminated about the causes of this disease. In our study we were aimed to investigate the morphometric alterations of orbitofrontal cortex in the somatization patients to understand the pathophsiology of the disorder.Volumes of the orbitofrontal cortex were measured by magnetic resonance imaging (MRI) in a sample of patients with DSM-IV SD and healty comparison subjects. In SD patients, the OFC volumes in both right and left sides were statistically significantly reduced compared to those of healthy controls.Our results consider that OFC has an important relationship with both pathophysiology and clinical course of the disease. However, it require new investigations with much larger sample particularly consisting of cognitive tests apart from magnatic resonance imaging, as we performed here.
