DENEYSEL OLARAK NONALKOLİK STEATOHEPATİT OLUŞTURULMUŞ RATLARDA TRİBULUS TERRESTRİSİN KARACİĞER VE ENDOTEL FONKSİYONLARI ÜZERİNE KORUYUCU ETKİSİNİN ARAŞTIRILMASI

Abstract

Kompleks mekanizmalar ve farklı etiyolojik nedenlere bağlı olarak gelişen NASH tablosunda NO, ADMA, MMP ve adhezyon moleküllerinin etkili olduğu ve özellikle kronikleşme süreci ile komplikasyonların gelişiminde önemli rol oynayabileceği düşünülmektedir. Biz bu çalışmada; Tribulus terrestris'in, NASH gelişimi ve kronikleşme sürecindeki mekanizmalara ve endotel fonksiyonlar üzerine etkilerini araştırmayı amaçladık. Çalışmada 200-220 gr arasında 35 adet erkek Sprague-Dawley cinsi ratlar kullanıldı. Ratlar her bir grupta 7 rat olacak şekilde 5 gruba ayrıldı. Grup1; kontrol gurubu (8 hafta), Grup2; fruktoz ile NASH oluşturulan grup (8 hafta), Grup3; NASH oluşumu sırasında Tribulus terrestris verilen grup (8 hafta), Grup4; NASH oluştuktan sonra standart beslenme (14 hafta), Grup5; NASH oluştuktan sonra Tribulus terrestris verilen grup (14 hafta). Çalışma tamamlandıktan sonra ratlar dekapite edilerek alınan kan örneklerinde elde edilen numunlerde biyokimyasal parametreler çalışıldı. Ratlardan alınan karaciğer doku örnekleri ve damar örnekleri de histopatolojik incelemeye tabi tutuldu. Çalışmada serum glukoz, AST, ALT, Total kolestrol, HDL, LDL, ICAM-1, VCAM-1, MMP-2, MMP-9, NO, ADMA düzeyleri ölçüldü ve Tribulus terrestris bitkisinin bu parametreler üzerine etkileri incelendi. MMP-2, MMP-9 ve ICAM-1 düzeyleri NASH oluştuktan sonra Tribulus terrestris bitki eksraktı verdiğimiz gruplarda anlamlı olarak (p<0.05) düşük tespit edilmiştir. NO ve ADMA düzeyleri NASH oluşumu sırasında Tribulus terrestris bitki ekstratı verdiğimiz gruplarda anlamlı olarak (p<0.05) düşük tespit edilmiştir. VCAM-1 düzeyleri ise gruplar arasında anlamlı bir farklılık olmadığı gözlenmiştir. Tribulus terrestris bitki ekstratı verdiğimiz gruplar, kontrol grubu ve NASH oluşturulan gruplarla kıyaslandığında ratlara verdiğimiz bitki eksraktının, karaciğer fonksiyon testlerini düzelttiği, ICAM-1, MMP-2, MMP-9, NO ve ADMA düzeylerini azalttığı, ayrıca fruktoz ile oluşturulan karaciğer hasarının histopatolojik olarak gerilediği gözlendi. Sonuç olarak, Tribulus terrestris bitki ekstratının karaciğer ve endotel fonksiyonları üzerine olumlu etki yapabildiği ve NASH'li hastaların etyopatogenezinde yararlı olabileceği kanısına varılmıştır.In the light of all these data, in the NASH table, occured depending on complex mechanisms and several aetiologic reasons, NO, ADMA, MMP and adhesion molecules are thought to be effective and play an important role on becoming chronic and on development of complication. In this study, we aimed to research the effects of Tribulus terrestris on NASH progress and on mechanisms and endothelium functions in the period of becoming chronic. In the study, 35 male Sprague-Dawley rats about weight of 200-220 g have been used. The rats were seperated to 5 groups, each of which has 7 rats. Group1; control group (8 weeks), Group2; NASH with fructose (8 weeks), Group3; given Tribulus terrestris during NASH progress (8 weeks), Group4; standard feding after NASH (14 weeks), Group5; given Tribulus terrestris after NASH progress (14 weeks). Rats were decapited after the study. Blood samples were taken and some biochemical parameters were studied. Liver, tissue and vessel samples taken from the rats were examined in the point of histopathology. The glucose, AST, ALT, total cholesterol, HDL, LDL, ICAM-1, VCAM-1, MMP-2, MMP-9, NO and ADMA levels were measured in the serum and the effects of Tribulus terrestris on these parameters were examined. After the formation of NASH, that we gave the Tribulus terrestris plant extract groups MMP-2, MMP-9 and ICAM-1 levels significantly different (p<0.05) were lower. During the formation of NASH, that we gave the Tribulus terrestris plant extract grops NO and ADMA levels significantly different (p<0.05) were lower. VCAM-1 levels significantly different between the groups weren't observed. We offer Tribulus terrestris plant extract groups, compared with the control group and created NASH group we give the plant extract to rats, which corrects liver function tests, ICAM-1, MMP-2, MMP-9, NO and ADMA levels reduce, also decreased histopathological confimation of fructose induced liver damage was observed. Consequently, Tribulus terrestris plant extract have a positive effect on the liver and endothelial function which can be useful to the etiopathogenesis of patients with NASH and it looks like a promising option has been concluded.
