FEMUR BAŞI AVASKÜLER NEKROZU OLAN HASTALARDA CORE DEKOMPRESYON CERRAHİSİ SONRASI KLİNİK SONUÇLARIMIZ

Abstract

Fırat Üniversitesi Tıp Fakültesi hastanesinde 2015-2021 yılları arası femur başı avasküler nekrozu ile başvuran kor dekompresyon cerrahisi yaptığımız toplam 27 hastanın 35 kalçası retrospektif olarak incelendi. Hastaların yaş ortalamaları 45,8' idi. Hastalarda en sık neden steroid kullanımı olarak bulundu. Hastaların ortalama takip süresi 36,1 ay olarak saptandı. Radyolojik değerlendirmede Ficat Arlet sınıflandırılması kullanıldı. Klinik değerlendirmede Harris Kalça skoruna ve Merle D'aubigne Postel Skoruna göre yapıldı. Kalçaların 1 tanesi ameliyat öncesi Ficat evre 1 olarak bulunurken, 29 tanesi evre 2, 5 tanesinin ise evre 3 olduğu belirlendi. Ameliyat sonrası ficat evrelenmesinde ise 21 kalçanın evre 2, 10 kalçanın evre 3 ve 4 kalçanın da evre 4 olduğu anlaşıldı. Hastaların ameliyat öncesi Harris kalça skoru en düşük 30, en yüksek 76 ortalama 41,34 olarak bulunurken ameliyat sonrası Harris kalça skoru ise en düşük 55, en yüksek 100 ortalama 82,85 puan olarak bulundu. Ameliyat öncesi Merle D'Augbine Postel Skoru en düşük 5, en yüksek 13 ortalama 7,9 iken ameliyat sonrası en düşük 10, en yüksek 18 ortalama 15 olarak bulundu. Bu çalışmada radyolojik olarak kor dekompresyon cerrahisi sonrası ilerlemenin devam ettiği fakat Harris ve Merle skoruna göre klinik olarak bariz iyileşmenin olduğu ortaya çıkmıştır.

A total of 27 patients who underwent core decompression surgery with avascular necrosis of the femoral head between 2015 and 2021 at Fırat University Faculty of Medicine Hospital were retrospectively analyzed. The mean age of the patients was 45.8 years. The most common reason in patients was found to be steroid use. The mean follow-up period of the patients was 36.1 months. The ficat arlet classification was used for radiological evaluation. Clinical evaluation was made according to Harris Hip score and Merle D'aubigne Postel Score. One of the patients was found to be preoperative stage 1, 29 of them were stage 2, and 5 of them were found to be stage 3. In the postop Ficat staging, 21 patients were stage 2, 10 patients were stage 3, and 4 patients were stage 4. The preop harris hip score of the patients was found to be the lowest 30 and the highest 76, with an average of 41.34, the postop harris hip score as the lowest 55, the highest 100, and the average 82.85. Preop Merle D'Augbine Postel Score was found to be preop lowest 5, highest 13 average 7.9, postop lowest 10 and highest 18 average 15. In this study, it was revealed radiologically that progress continued after core decompression surgery, but clinically obvious improvement was observed according to the harris and merle score.
