OTOJEN HAMSTRİNG TENDONLARIYLA ARTROSKOPİK ÖN ÇAPRAZ BAĞ REKONSTRÜKSİYONU CERRAHİSİNİN SONUÇLARININ DEĞERLENDİRİLMESİ

Abstract

Bu çalışmada semptomatik kronik ön çapraz bağ yırtığının dört katlı otojen Hamstring tendon grefti kullanılarak yapılan artrosko¬pik rekonstruksiyonunun erken dönem sonuçları incelenmiştir.2008 ? 2010 tarihleri arasında Fırat Üniversitesi Tıp Fakültesi Hastanesi Ortopedi ve Travmatoloji Anabilim Dalında artroskopik olarak ön çapraz bağ yırtığı rekonstrüksiyonu yapılan 76 hasta incelendi. Tüm hastalar ortalama 25,6 (15 ? 37) ay sonunda fonksiyonel olarak Lysholm diz skorlaması, IKDC (International Knee Documentation Committee) skorlaması ve Tegner aktivi¬te skorlaması ile klinik olarak diz eklem hareket açıklığı, stabilite testleri, uyluk çevresi ölçümü ve kişisel memnuniyet ile değerlendirildi.Ameliyat öncesi Lysholm skoru ortalama 64,3 iken ameliyat sonrası son kontrolde 93,2 idi. IKDC skoruna göre hastaların ameliyat öncesi 74'ünde (% 97,36) sonuçlar C ve D olarak, ameliyat sonrasında ise 68'inde (% 89,48) sonuçlar A ve B, 8'inde (% 10,52) ise C olarak değerlendirildi.Cerrahi tedavi kararı alınırken hastanın yaşam tarzı ve beklentileri iyi anlaşılmalıdır. Ön çapraz bağ rekonstrüksiyonu yapılan hastalarda en az cerrahi kadar önemli olan bir unsur da rehabilitasyon programıdır. Hastaya bunun en az cerrahi kadar önemli olduğu anlatılmalıdır.Ön çapraz bağ yırtıklarının, dört katlı otojen Hamstring tendon grefti kullanılarak yapılan artroskopik rekonstrüksiyonlarında hem fonksiyonel olarak düzelme hem de hastalarda yüksek memnuniyet oranları tespit edilmiştir.Anahtar Kelimeler: Ön çapraz bağ, otojen Hamstring tendon grefti, artroskopik rekonstrüksiyon.

We evaluated the early results of arthroscopic reconstruction of anterior cruciate ligament (ACL) ruptures using autogenous four strand hamstring tendon graft.We evaluated 76 patients undergo arthroscopic reconstruction of ACL tears whom are treated surgically between years 2008 ? 2010 in Fırat University Hospital Department of Orthopaedics Clinics.All patients were evaluated with Lysholm knee score, IKDC (International Knee Documentation Committee) score and Tegner activity scoring for functional evaluation, range of motion of the joint, stability tests, thigh atropy measurements and subjective satisfaction for clinical evaluation at the end of the postoperative 25,6 (15 ? 37) month.Preoperative Lysholm score average was 64,3 and improved 93,2 at the end of the follow-up. According to the IKDC; preop 74 (% 97,36) pateints were graded C and D, postop 68 (% 89,48) pateints were graded A and B, 8 (% 10,52) pateints were graded C.The patient?s life style and expectations must be understood before deciding the surgery. The rehabilitation programmes are as important as the surgery performed. Patients must be awared of this.We conclude that arthroscopic reconstructions of anterior cruciate ligament with use of a four strand Hamstring tendon autograft which revealed successful functional results and also provided high personal satisfaction, is a reliable technique in the surgical treatment of anterior cruciate ligament ruptures.Key words: Anterior cruciate ligament, autogenous Hamstring tendon graft, arthroscopic reconstruction.
