Bu çalışmanın amacı, eklemeli ve eksiltmeli dijital üretim yöntemleriyle üretilen geçici kron restorasyonlarının marjinal uyum, internal uyum ve kırılma dayanımı açısından karşılaştırmalı olarak değerlendirilmesidir. Çalışmada, dört farklı materyal grubu kullanılarak toplam 40 adet geçici kron restorasyonu üretilmiştir. Her bir materyal grubunda 10 adet kron örneği yer almıştır. Eklemeli üretim grubunda 3D baskı yöntemiyle üretilen Formlabs ve NextDent materyalleri; eksiltmeli üretim grubunda ise CAD/CAM frezeleme yöntemiyle üretilen VITA CAD-Temp ve Yamahachi materyalleri değerlendirilmiştir. Tüm örnekler dijital olarak tasarlanmış; eklemeli üretim grupları 3D baskı yöntemiyle, eksiltmeli üretim grupları ise CAD/CAM frezeleme yöntemiyle elde edilmiştir. Marjinal ve internal uyum değerlendirmeleri, triple-scan tekniği ile elde edilen dijital veriler üzerinden gerçekleştirilmiştir. Ölçümler, Geomagic Control X yazılımında alınan belirli kesitler üzerinde "2D Dimension" komutu kullanılarak yapılmış; marjinal, aksiyal ve oklüzal bölgelerde kron–güdük arasındaki boşluk değerleri kaydedilmiştir. Kırılma dayanımı değerlendirmesi öncesinde örnekler termal siklus işlemine tabi tutulmuş, ardından üniversal test cihazında yükleme uygulanarak kırılma dayanımı değerleri belirlenmiştir. Elde edilen bulgular, hem üretim yöntemi hem de materyal grubu açısından marjinal uyum, internal uyum ve kırılma dayanımı değerlerinde istatistiksel olarak anlamlı farklılıklar bulunduğunu göstermiştir. Üretim yöntemi açısından değerlendirildiğinde, frezeleme yöntemiyle üretilen restorasyonların marjinal uyum ortalaması, 3D baskı yöntemiyle üretilen restorasyonlara göre istatistiksel olarak anlamlı düzeyde daha yüksek bulunmuştur. Benzer şekilde, internal uyum değerlendirmesinde de frezeleme yöntemiyle üretilen restorasyonların boşluk değerleri 3D baskı gruplarına kıyasla anlamlı derecede yüksek saptanmıştır. Bu bulgular, 3D baskı yöntemiyle üretilen geçici kron restorasyonlarının marjinal ve internal uyum açısından daha düşük boşluk değerleri sergilediğini göstermektedir. Buna karşılık, kırılma dayanımı açısından frezeleme yöntemiyle üretilen restorasyonların 3D baskı gruplarına göre anlamlı derecede daha yüksek değerlere sahip olduğu belirlenmiştir. Materyal grupları kendi içinde değerlendirildiğinde, marjinal uyum açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmıştır. Marjinal uyumda özellikle VITA CAD-Temp grubunun boşluk değerleri, NextDent grubuna kıyasla daha yüksek bulunmuştur. İnternal uyum değerlendirmelerinde de materyaller arasında anlamlı fark belirlenmiş; VITA CAD-Temp grubunun internal uyum ortalaması, Formlabs ve NextDent gruplarına göre istatistiksel olarak anlamlı düzeyde yüksek saptanmıştır. Buna karşılık, Formlabs ve NextDent grupları internal uyum açısından daha düşük boşluk değerleri sergilemiştir. Kırılma dayanımı bulguları incelendiğinde, materyal grupları arasında istatistiksel olarak anlamlı fark bulunmuştur. VITA CAD-Temp grubu en yüksek kırılma dayanımı değerlerini gösterirken, NextDent grubu tüm diğer gruplardan anlamlı derecede düşük kırılma dayanımı sergilemiştir. Formlabs ve Yamahachi grupları arasında ise kırılma dayanımı açısından istatistiksel olarak anlamlı fark saptanmamıştır. Bu in vitro çalışmanın sınırları dahilinde, eklemeli ve eksiltmeli yöntemlerle üretilen geçici kron restorasyonlarının marjinal/internal uyumları ve kırılma dayanımlarının üretim yöntemi ve materyal grubuna bağlı olarak değiştiği sonucuna varılmıştır. 3D baskı yöntemiyle üretilen restorasyonlar marjinal ve internal uyum açısından daha avantajlı sonuçlar verirken, CAD/CAM frezeleme yöntemiyle üretilen restorasyonlar kırılma dayanımı açısından daha yüksek performans göstermiştir. Bu sonuçlar, dijital yöntemlerle geçici restorasyon üretiminde yalnızca üretim yönteminin değil, kullanılan materyalin de klinik başarı açısından belirleyici olduğunu göstermektedir. Bu nedenle materyal ve yöntem seçiminin, restorasyondan beklenen uyum ve mekanik dayanım gereksinimleri dikkate alınarak yapılması gerektiği düşünülmektedir.
The aim of this study was to comparatively evaluate the marginal fit, internal fit, and fracture resistance of provisional crown restorations fabricated using additive and subtractive digital manufacturing methods. In this study, a total of 40 provisional crown restorations were fabricated using four different material groups, with 10 crown specimens in each group. Formlabs and NextDent materials were evaluated in the additive manufacturing group using the 3D printing method, while VITA CAD-Temp and Yamahachi materials were evaluated in the subtractive manufacturing group using the CAD/CAM milling method. All specimens were digitally designed; the additive manufacturing groups were produced by 3D printing, whereas the subtractive manufacturing groups were fabricated by CAD/CAM milling. Marginal and internal fit evaluations were performed using digital data obtained with the triple-scan technique. Measurements were carried out on the sections obtained in Geomagic Control X software using the "2D Dimension" command, and the gaps between the crown and die were recorded in the marginal, axial, and occlusal regions. Before fracture resistance testing, the specimens were subjected to thermocycling, and fracture resistance values were then determined by applying load using a universal testing machine. The findings showed statistically significant differences in marginal fit, internal fit, and fracture resistance values in terms of both manufacturing method and material group. When evaluated according to the manufacturing method, the mean marginal gap values of restorations produced by the milling method were found to be statistically significantly higher than those of restorations produced by the 3D printing method. Similarly, in the internal fit evaluation, the gap values of restorations produced by the milling method were significantly higher than those of the 3D-printed groups. These findings indicate that provisional crown restorations produced by 3D printing exhibited lower gap values in terms of marginal and internal fit. In contrast, restorations produced by the milling method showed significantly higher fracture resistance values than the 3D-printed groups. When the material groups were evaluated individually, a statistically significant difference was found among the groups in terms of marginal fit. In particular, the marginal gap values of the VITA CAD-Temp group were higher than those of the NextDent group. Significant differences were also observed among the materials in the internal fit evaluation; the mean internal gap value of the VITA CAD-Temp group was statistically significantly higher than those of the Formlabs and NextDent groups. In contrast, the Formlabs and NextDent groups exhibited lower gap values in terms of internal fit. Regarding fracture resistance, a statistically significant difference was found among the material groups. The VITA CAD-Temp group showed the highest fracture resistance values, whereas the NextDent group exhibited significantly lower fracture resistance than all other groups. No statistically significant difference was found between the Formlabs and Yamahachi groups in terms of fracture resistance. Within the limitations of this in vitro study, it was concluded that the marginal/internal fit and fracture resistance of provisional crown restorations fabricated using additive and subtractive methods varied depending on the manufacturing method and material group. While restorations produced by 3D printing showed more favorable results in terms of marginal and internal fit, restorations produced by CAD/CAM milling demonstrated higher performance in terms of fracture resistance. These results indicate that not only the manufacturing method but also the material used is a determining factor for clinical success in the digital fabrication of provisional restorations. Therefore, material and method selection should be made by considering the fit and mechanical strength requirements expected from the restoration.
