| dc.description.abstract | Mizofoni, belirli seslere karşı yoğun sıkıntı, öfke ve davranışsal tepkilerle seyreden, klinikte sık gözlenen ancak henüz Ruhsal Bozuklukların Tanısal ve İstatistiksel El Kitabı'nda (DSM) yer almayan bir durumdur. Etiyolojisine yönelik nörobiyolojik veriler sınırlı olduğundan, bu çalışmada yapısal manyetik rezonans görüntüleme (MRG) ile beyin hacim farklılıklarının incelenmesi amaçlanmıştır. Çalışmaya Fırat Üniversitesi Hastanesi Psikiyatri Anabilim Dalı'na başvuran 31 mizofoni hastası ve 30 sağlıklı kontrol dâhil edilmiştir. Katılımcılara sosyodemografik ve klinik veri formu, WHOQOL-BREF, obsesif kompulsif kişilik bozukluğu (OKKB) Tarama Anketi, Mizofoni Belirti Listesi (MBL) ve Amsterdam Mizofoni Ölçeği (A-MISO-S) uygulanmıştır. Yapısal MRG ile orbitofrontal korteks (OFK), amigdala, anterior singulat korteks (ASK) ve talamus hacimleri ölçülmüştür. Gruplar arasında söz konusu beyin bölgelerinin hacimleri açısından istatistiksel olarak anlamlı farklılık saptanmamıştır. Bununla birlikte vaka grubunda, OKKB tarama ölçeği puanları ile sol ASK, bilateral talamus ve serebrum hacimleri arasında negatif korelasyon bulunmuştur. Vaka grubunda A-MISO-S puanları ise sağ OFK, sağ ASK, bilateral talamus, serebrum ve total beyin hacimleriyle negatif korelasyon göstermiştir. Ayrıca vaka grubunda sosyodemografik değişkenlerle ilişkili bazı beyin bölgelerinde anlamlı hacim farklılıkları gözlenmiştir. Bu bulgular, mizofoni belirtilerinin yalnızca klinik düzeyde değil, aynı zamanda nörobiyolojik ve sosyodemografik faktörlerle de ilişkili olabileceğini göstermektedir. Sonuç olarak, çalışmamız mizofoninin nörobiyolojik mekanizmalarının ve klinik yansımalarının anlaşılmasına katkı sunmakta, sosyodemografik değişkenlerin de bu süreçte dikkate alınması gerektiğini ortaya koymaktadır. Bu yönüyle, geniş örneklemli ve çok merkezli ileri araştırmalar için öncü bir nitelik taşımaktadır. | dc.description.abstract | Misophonia is characterized by intense distress, anger, and behavioral reactions to specific sounds, and although it is frequently observed in clinical practice, it is not yet included in the Diagnostic and Statistical Manual of Mental Disorders (DSM). As neurobiological data on its etiology are limited, this study aimed to investigate brain volume differences using structural magnetic resonance imaging (MRI). The study included 31 patients with misophonia and 30 healthy controls who presented to the Department of Psychiatry at Fırat University Hospital. Participants completed a sociodemographic and clinical data form, WHOQOL-BREF, the Obsessive-Compulsive Personality Disorder (OCPD) Screening Questionnaire, the Misophonia Symptom Scale (MBL), and the Amsterdam Misophonia Scale (A-MISO-S). Structural MRI was used to measure the volumes of the orbitofrontal cortex (OFC), amygdala, anterior cingulate cortex (ACC), and thalamus. No statistically significant differences were found between the groups in the volumes of these brain regions. However, in the patient group, negative correlations were observed between OCPD screening scores and the volumes of the left ACC, bilateral thalamus, and cerebrum. In addition, A-MISO-S scores showed negative correlations with the right OFC, right ACC, bilateral thalamus, cerebrum, and total brain volumes. Furthermore, significant volumetric differences were found in certain brain regions in association with sociodemographic variables within the patient group. These findings suggest that misophonia symptoms may be related not only to clinical features but also to neurobiological and sociodemographic factors. In conclusion, our study contributes to the understanding of the neurobiological mechanisms and clinical implications of misophonia, highlighting the importance of considering sociodemographic variables in this process. In this respect, it serves as a preliminary step for future large-sample and multicenter studies.
