TOTAL ABDOMİNAL HİSTEREKTOMİ OLGULARINDA QUADRATUS LUMBORUM BLOĞUNUN POSTOPERATİF ANALJEZİ ÜZERİNE ETKİSİNİN DEĞERLENDİRİLMESİ

Abstract

Bu çalışmada, total abdominal histerektomi uygulanan hastalarda quadratus lumborum bloğunun (QLB) postoperatif analjezi üzerine etkisinin değerlendirilmesi amaçlandı. Genel anestezi altında abdominal histerektomi operasyonu yapılan ve postoperatif analjezi amacıyla QLB uygulanan ve uygulanmayan toplam 50 hasta çalıs?maya alındı. Her iki grupta hastaların yas?ı, ağırlık, boy, ASA skorları, operasyon süresi, postoperatif derlenme ünitesindeki kalp atım hızı, sistolik ve diyastolik kan basınçları,1., 2., 6., 12 ve 24. Saat Visüel Analog Skala (VAS) değerleri, ek analjezi gereksinimleri, herhangi bir komplikasyon veya yan etki olup olmadığı, hasta ve cerrah memnuniyeti değerleri karşılaştırıldı. Postoperatif 1., 2., 4., 12., 24. saat VAS değerleri ve postoperatif ilk 24 saat opioid kullanımı grup k (kontrol) da istatiksel olarak anlamlı derecede yüksekti. Hasta ve cerrah memnuniyeti grup blok da istatiksel olarak anlamlı derecede yüksekti. Her iki grubun postoperatif kan basınçları, kalp atım hızı, ameliyat süresi, ASA skorları, operasyon süreleri benzerdi. Çalışmamızda QLB uygulanan hastalarda postoperatif ilk 24 saatte tüketilen analjezik miktarının azaldığı, VAS skorlarının düştüğü, hasta ve cerrah memnuniyetinin arttığı saptandı. Ultrason eşliğindeki QLB yapılmış abdominal cerrahi uygulanan hastalarda, postoperatif dönemde ağrıyı gidermek ve opioid gereksinimini azaltmak için alternatif, güvenli ve etkili bir yöntem olduğu dolayısıyla QLB uygulamasının geleneksel analjezik yöntemlere alternatif olduğu düşüncesindeyiz. Ancak literatürde sınırlı hasta ve çalışma olmasi nedeniyle hasta ve çalışma sayısı artması gerektiği sonucuna varılmıştır. In this study, it was aimed to evaluate the effect of quadratus lumborum block (QLB) on postoperative analgesia in patients who underwent total abdominal hysterectomy. A total of 50 patients who underwent abdominal hysterectomy under general anesthesia and underwent QLB surgery and underwent abdominal hysterectomy surgery were included in the study. The age, weight, height, sagittal scores, postoperative heart rate, systolic and diastolic blood pressures, 1st, 2nd, 6th, 12th and 24th hour VAS values, additional analgesia requirement, complications or side effects, patient and surgeon satisfaction values were compared. Postoperative 1., 2., 4., 12., 24. hour VAS values and opioid use in the first 24 hours postoperatively were significantly higher in group K( control). Patient and surgeon satisfaction was significantly higher in group QLB. Postoperative blood pressures, heart rate, duration of operation, ASA, operation time were similar in both groups. In our study, it was determined that the patients who were treated with QLB had decreased the amount of analgesic consumed in 24 hours, the VAS scores decreased, and patient and surgeon satisfaction increased. We believe that QLB is an alternative to traditional analgesic methods because it is an alternative, safe and effective method for reducing pain and opioid withdrawal in patients undergoing ultrasound-assisted abdominal surgery. However, due to the limited number of patients and studies in the literature, it has been concluded that the number of patients and the number of studies should increase.
